血液透析联合rHu EPO对尿毒症贫血患者Hb、Hct水平的影响

摘　　要：目的研究血液透析联合重组人红细胞生长素(rHu EPO)对尿毒症贫血患者血红蛋白(hemoglobin,Hb)、红细胞比容(hematocrit,Hct)水平的影响。方法本文选取2016年8月至2018年8月就诊的尿毒症患者90例,按照随机数字表法分为联合组和血液透析组,每组45例。血液透析组给予血液透析治疗,联合组在血液透析组的基础上给予rHu EPO治疗。检测患者Hb、Hct、血清铁蛋白、铁转蛋白饱和度、血肌酐、血尿素、血尿素氮、C-反应蛋白(C-reactive protein,CRP)、白介素-6(Interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平,统计治疗有效率、并发症发生率。结果2组治疗后Hb、Hct、血清铁蛋白、铁转蛋白饱水平高于治疗前(P<0.05)。联合组治疗后Hb、Hct、血清铁蛋白、铁转蛋白饱水平高于血液透析组治疗后(P<0.05)。2组治疗后血尿素氮、血尿素、血肌酐、CRP、IL-6、TNF-α水平低于治疗前(P<0.05)。联合组治疗后血尿素氮、血尿素、血肌酐、CRP、IL-6、TNF-α水平低于血液透析组治疗后(P<0.05)。联合组治疗有效率为93.33%高于血液透析组患者治疗有效率77.78%(P<0.05)。结论血液透析联合rHu EPO治疗尿毒症贫血能改善患者贫血症状,改善患者肾功能,降低炎性反应,提供治疗效果。Objective To investigate the effects of hemodialysis combined with recombinant human erythrocyte growth factor(rHu EPO)on the levels of hemoglobin(Hb)and hematocrit(Hct)in patients with uremic anemia.Methods Ninety patients with uremia who were treated in our hospital from August 2016 to August 2018 were enrolled in the study,who were divided into two groups according to random number table,with 45 cases in each group.The patients in hemodialysis group were treated by hemodialysis,however,the patients in combination treatment group,EPO on the basis of hemodialysis group,were treated by rHu EPO.The levels of Hb,Hct,serum ferritin,transferin saturation,serum creatinine,blood urea,blood urea nitrogen,C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)were detected,moreover,the effective rate and the incidence rate of complications were observed and compared between the two groups.Results After treatment,the levels of Hb,Hct,serum ferritin and transferin saturation in both groups were significantly higher than those before treatment(P<0.05),moreover,which in combination treatment group were significantly higher than those in hemodialysis group(P<0.05).After treatment,the levels of blood urea nitrogen,blood urea,creatinine,CRP,IL-6 and TNF-αin both groups were significantly lower than those before treatment(P<0.05),moreover,which in combination treatment group were significantly lower than those in hemodialysis group(P<0.05).In addition the effective rate in combination treatment group was 93.33%,which wa significantly higher than that(77.78%)in hemodialysis group(P<0.05).Conclusion Hemodialysis combined with rHu EPO in treatment of uremic anemia can improve the symptoms,improve renal function,reduce inflammation reaction of patients,with satisfactory therapeutic effects.
