摘　　要：目的:通过对肺间质性疾病患者的胸部CT特征分析及CT分级的初步探索,探讨CT分级对评估肺间质性疾病严重程度的价值。方法:回顾性分析38例经临床诊断为肺间质性疾病患者的胸部CT,并根据CT分级标准进行分级。男25例,女13例,平均年龄65±10。所有病例进行HRCT扫描,由放射科高年资医生对CT图像进行分析、总结。结果:间质性肺疾病患者胸部CT的影像表现有磨玻璃样改变、肺实变、小叶间隔增厚、细网格状影、网格状影、牵拉性支气管扩张、蜂窝影、条索状影、结节影、胸膜增厚等,其中:磨玻璃影(23例),实变影(11例),小叶间隔增厚(37例),网格状影(21例),蜂窝影(13例)。CT分级:轻度(6例),中度(19例),重度(13例)。结论:通过对肺间质性疾病患者CT征象的分析与分级,对临床判断患者疾病严重程度及治疗方向有一定的作用。Objective:The value of CT classification in evaluating the severity of interstitial lung disease was discussed by analyzing the chest CT features and preliminary exploration of CT classification in patients with interstitial lung disease(ILD).Methods:Chest CT imaging data of 38 ILD patients were retrospectively analyzed and classified according to the CT classification criteria.There were 25 males and 13 females,with the mean age of 65±10.All cases underwent high-resolution CT scan,and the CT images were analyzed and summarized by senior radiologists.Results:The chest CT image of the 38 ILD patients include GGO,lung consolidation,interlobular septal thickening,fine mesh shadow,mesh shadow,tractable bronchiectasis,honeycombhadow,stripyshadow,nodule shadow,pleural thickening.Amongthem:ground glass shadow(23 cases);Consolidation shadow(11 cases)and interlobular septal thickening(37 cases).Grid-like shadow(21 cases).CT classification:mild(6 cases);Moderate(19 cases);Severe(13 cases).Conclusion:It is helpful to judge the severity of the disease and direction of treatment by analyzing and classifying the CT signs for the patients with interstitial lung disease.
