摘　　要：目的观察不同浓度肝素封管液运用于老年病人中心静脉导管封管的效果,为临床上老年病人使用最佳浓度的封管液提供理论依据。方法将495例留置中心静脉导管的老年病人随机分成A、B、C组,每组165例。A、B、C组分别采用10.00 U/mL、31.25 U/mL、62.50 U/mL浓度的肝素液进行正压封管。观察3组病人留置中心静脉导管的堵管发生率,封管前后PLT、凝血5项指标水平并比较。结果3组中心静脉导管堵管发生率及封管前后PLT比较,差异均无统计学意义(P>0.05)。C组封管前后的凝血酶原时间(PT)、凝血酶原活动度(PA)差异具有统计学意义(P<0.05),其余2组凝血5项水平差异均无统计学意义(P>0.05)。结论10.00 U/mL肝素封管液对老年病人影响小,可以作为老年病人中心静脉导管封管使用标准。Objective To observe the effect of different concentrations of heparin sealing fluid on central venous catheter sealing in the elderly patients,and to provide theoretical basis for the use of the best concentration of sealing fluid.Methods A total of 495 elderly patients with central venous catheter were randomly divided into group A,B and C,with 165 cases in each group.In group A,B and C,heparin solution with concentration of 10.00 U/mL,31.25 U/mL and 62.50 U/mL was used for positive pressure sealing,respectively.The incidence of central venous catheter occlusion,platelet count(PLT)and coagulation function before and after catheter closure were observed and compared.Results There was no significant difference in the incidence rate of central venous catheter occlusion among the three groups(P>0.05).There was no significant difference in the level of PLT among the three groups(P>0.05).There were significant differences in prothrombin time(PT)and prothrombin activity(PA)before and after sealing in group C(P<0.05).Conclusions 10 U/mL heparin sealing solution shows few effect on the elderly patients,and it can be used as the standard for sealing the central venous catheter in the elderly patients.
