去乙酰毛花苷注射液单次静推联用芪苈强心胶囊、螺内酯片致洋地黄中毒一例

摘　　要：本文报道1例肾功能不全的高龄老年患者应用去乙酰毛花苷注射液(单次静推0.2 mg)联合芪苈强心胶囊、螺内酯片治疗致洋地黄中毒,地高辛血药浓度最高为4.36 ng/ml,治疗期间未使用其他洋地黄制剂,17 d后患者血药浓度仍达到2.17 ng/ml。单用1次洋地黄制剂致洋地黄中毒持续如此长时间在国内罕见报道,对于肾功能不全的患者,尤其是联合影响洋地黄血药浓度的药物应谨慎使用。Objective A case of an elderly patient with renal insufficiency who developed digitalis poisoning induced by single static push of acetyl deslanoside injection 0.2 mg and oral use of qiliqiangxin capsule was reported.The highest drug concentration of digoxin of the patient was 4.36 mg/ml,and it was 2.17 mg/ml after 17 d without other digitalis agents.Such a long duration for single use of digitalis agents-induced digitalis poisoning is rarely reported in China,which suggested that combination use with drugs that can affect digitalis agents should be cautious for patients with renal insufficiency.
