骨质疏松症合并高尿酸血症患者维生素D水平与骨代谢、血尿酸的关系

摘　　要：目的探讨骨质疏松症合并高尿酸血症患者维生素D水平与骨代谢、血尿酸(BUA)的关系。方法选取72例骨质疏松症合并高尿酸血症者患者作为合并疾病组、85例单纯骨质疏松症者患者作为单纯疾病组及60例体检健康者作为健康组。所有受试者均检测血清25羟维生素D3(25-OHD3)、骨钙素(BGP)、β-Ⅰ型胶原羧基端交联肽(β-CTX)、Ⅰ型前胶原氨基端前肽(PINP)、骨特异性碱性磷酸酶(BALP)、抗酒石酸酸性磷酸酶-5b(TRACP-5b)、BUA水平,并分析合并疾病组25-OHD3与骨代谢指标、BUA的关系。结果血清25-OHD3、BALP水平在合并疾病组<单纯疾病组<健康组,BGP、β-CTX、PINP、TRACP-5b水平在合并疾病组>单纯疾病组>健康组,差异均有统计学意义(P<0.05);合并疾病组BUA水平明显高于单纯疾病组、健康组(P<0.05),单纯疾病组与健康组比较无明显差异(P>0.05);经Pearson相关性分析,合并疾病组25-OHD3与BGP、β-CTX、PINP、TRACP-5b、BUA均呈负相关,与BALP呈正相关(P<0.05)。结论骨质疏松症合并高尿酸血症患者血清25-OHD3水平降低,与BGP、β-CTX、PINP、TRACP-5b及BUA呈负相关性,而与BALP呈正相关性。Objective To explore the relationship between vitamin D and bone metabolism and blood uric acid(BUA)in patients with osteoporosis and hyperuricemia.Methods 72 cases of osteoporosis with hyperuricemia(combined disease group),85 cases of simple osteoporosis(simple disease group)and 60 healthy persons for physical examination(healthy group)were selected.The serum level of 25 hydroxylated vitamin D3(25-OHD3),osteocalcin(BGP),βcross-linked telepeptide of type-Ⅰcollagen(β-CTX),amino-terminal procollagen of type-Ⅰcollagen(PINP),bone specific alkaline phosphatase(BALP),tartrate resistant acid phosphatase-5b(TRACP-5b)and BUA were measured,then the relationship between 25-OHD3 and bone metabolism index and BUA in combined disease group were analyzed.Results The serum levels of 25-OHD3 and BALP in the combined disease group<which in the simple disease group<which in the healthy group,and the levels of BGP,β-CTX,PINP and TRACP-5b in the combined disease group>which in the simple disease group>which in the healthy group,and the differences were statistically significant(P<0.05).The level of BUA in the combined disease group was significantly higher than that in the simple disease group and the healthy group(P<0.05),but there was no significant difference in BUA level between the simple disease group and the healthy group(P>0.05).By Pearson correlation analysis,the 25-OHD3 level in the combined disease group was negatively correlated with BGP,β-CTX,PINP,TRACP-5b and BUA,but it was positively correlated with BALP(P<0.05).ConclusionThe level of serum 25-OHD3 in osteoporosis with hyperuricemia is decreased,which is negatively correlated with BGP,β-CTX,PINP,TRACP-5b and BUA,but it is positively correlated with BALP.
