摘  要:目的探讨3D打印技术用于微创治疗胫腓骨远近端复杂性骨折的临床效果及应用价值。方法回顾性分析2014年9月至2019年6月马鞍山市人民医院收治的胫腓骨远近端复杂性骨折患者22例。采用3D打印技术辅助微创技术塑性钢板内固定治疗11例(观察组),采用切开复位锁定钢板内固定治疗11例(对照组)。比较两组的手术时间、术中出血量、术中透视次数、切口长度和术后膝踝功能评分等。结果所有病例均获得随访,平均随访时间12.1(6~14)个月。观察组在手术时间、术中出血量、术中透视次数、切口长度较对照组少,差异有统计学意义(<0.05)。观察组术后膝关节功能恢复较对照组有显著差异(<0.01);踝关节的功能恢复及术后疼痛与对照组比较,差异无统计学意义(>0.05)。结论 3D打印技术可以使术前复杂的骨折移位更加直观,利于术前科学精准规划,结合塑形钢板内固定治疗胫腓骨远近端复杂性骨折,可以减少手术造成的损伤,有利于术后恢复。Objective To explore the clinical effect and application value of 3 D printing model combined with minimally invasive technology in the treatment of distal and proximal tibiofibular fracture.Methods Retrospective analysis of 22 patients with complex fractures of the distal and proximal tibia and fibula from September 2014 to June 2019.11 cases(observation group)were treated with precontoured plate internal fixation with 3 D printing model and minimally invasive technology.11 cases(control group)were treated with open reduction and locking plate internal fixation.The operation time,intraoperative blood loss,the number of intraoperative perspectives,incision length and the postoperative knee and ankle function scores of the two groups were compared.Results All cases were followed up,with an average followup time of 12.1(6-14)months.The observation group had less operative time,intraoperative blood loss,intraoperative X-irradiation time,and incision length than the control group.The differences were statistically significant(P<0.05).The knee joint function recovery in the observation group was significantly different than that in the control group(P<0.01),and the ankle joint function recovery and postoperative pain were not significantly different from those in the control group(P>0.05).Conclusion 3 D printing model can make complex fracture displacement more intuitive before surgery,facilitate scientific and accurate planning before surgery,and use precontoured plate internal fixation to treat complex fractures of the distal and proximal tibia and fibula.
