占位器在人工髋关节置换术后感染二期翻修中的应用及研究进展

摘　　要：人工髋关节置换术后感染对于每个患者来讲都属于灾难性的并发症,给患者精神及经济上造成很大压力,也给临床医师带来巨大挑战。随着对该并发症认识的逐步深化和治疗方式的改进,术后感染率的控制和关节功能的恢复有了显著提高。虽然目前治疗方式各异,但取出关节假体彻底清创,使用临时占位器避免软组织挛缩,为关节内提供高浓度抗生素,待无明显感染征象后再行二期手术翻修是目前治疗的共识。现多种型号占位器广泛应用于临床,但在国内外尚无统一的制作方法及标准,因此患者预后也出现了较大的差异性。本文通过对比不同占位器的力学性能和临床疗效,描述了占位器在髋关节置换术后感染二期翻修中的应用,为临床选择和应用提供指导。Infection after artificial hip arthroplasty is a catastrophic complication,which puts a lot of mental and economic stress on the patient and poses great challenges for clinicians.With the gradual deepening of the understanding of the complications and the improvement of treatment methods,the control of postoperative infection rate and the joint function of patients have been significantly improved.Although the current treatment methods are different,it is generally accepted that the joint prosthesis should be removed for thorough debridement,temporary spacers should be used to maintain the soft tissue tension,high-concentration antibiotics should be provided within the joint,and the second-stage revision should be performed after no obvious signs of infection.At present,a variety of clinical spacers are available for selection,but there is no uniform production method and standard for temporary spacer at home and abroad.Therefore,the prognosis of patients has also appeared a large difference.In this paper,the application of the spacer in the second-stage revision of the infection around the hip prosthesis is elaborated by comparing the mechanical characteristics and clinical effects of different placeholders,which provides a reference for clinical selection and application.
