摘 要:目的研究磁共振弥散张量成像(DTI)在颈椎管内肿瘤手术预后评估中的作用。方法选取2018年1月-2020年6月该院接受手术治疗的颈椎管内肿瘤患者30例为研究对象,于术前3 d和术后1个月对患者进行颈髓MRI常规序列和DTI扫描。分析患者各向异性分数(FA)和表观弥散系数(ADC)值,并分别对患者术前、术后3个区域的FA值和ADC值进行比较,观察患者的手术结果和神经脊髓功能改善情况。结果患者肿瘤头区和肿瘤区术后1个月的FA值、ADC值分别为(0.641±0.152)和(0.682±0.094)、(1.305±0.485)×10^-3mm^2/s和(1.382±0.319)×10^-3mm^2/s,与术前的(0.528±0.075)和(0.563±0.069)、(1.602±0.451)×10^-3mm^2/s和(1.578±0.426)×10^-3mm^2/s比较,差异有统计学意义(t=3.652、5.590、2.456、2.017,P<0.05);30例患者中肿瘤完全切除的患者有25例,占83.33%,总体改善率为72.09%。结论磁共振弥散张量成像在颈椎管内肿瘤诊断中的应用价值非常高。Objective To study the role of MRI diffusion tensor imaging(DTI)in the evaluation of the prognosis of cervical spinal tumor surgery.Methods Thirty patients with cervical spinal canal tumors who underwent surgical treatment in the hospital from January 2018 to June 2020 were selected as the research objects.The patients were subjected to routine cervical spinal MRI sequence and DTI scan 3 days before surgery and 1 month after surgery.Analyze the patient's anisotropy score(FA)and apparent diffusion coefficient(ADC)values,and compare the FA and ADC values in the three areas of the patient before and after the operation,and observe the patient's surgical results and improvement of neurospinal function.Results FA and ADC values were(0.641±0.152),(0.682±0.094),(1.305±0.485)×10^-3 mm^2/s and(1.382±0.319)×10^-3 mm^2/s,respectively with preoperative(0.528±0.075)and(0.563±0.069),(1.602±0.451)×10^-3 mm^2/s and(1.578±0.426)the differences were statistically significant(t=3.652,5.590,2.456,2.017,P<0.05);Of the 30 patients,25 were completely removed,83.33%,The overall improvement rate was 72.09%.Conclusion The application value of MRI diffusion tensor imaging in the diagnosis of cervical spinal tumors is very high.
