咪达唑仑与右美托咪定在老年患者四肢手术中的镇静效果与不良反应分析

摘　　要：目的探讨咪达唑仑与右美托咪定在老年患者四肢手术中的应用效果,结合患者的镇静效果及不良反应发生情况进行评价。方法方便选取2018年4月—2019年8月期间于该院接受四肢手术治疗的96例患者作为研究对象,应用单盲随机法,分为观察组(右美托咪定)和对照组(咪达唑仑),各48例,对比两组的镇静效果。结果与对照组相比,观察组患者的镇静效果优良率(95.83%>77.08%)相对更高,差异有统计学意义(χ^2=7.207,P<0.05),不良反应发生率(4.16%<16.66%)、呛咳次数[(0.15±0.07)次/min<(0.32±0.10)次/min]相对更低,苏醒时间[(22.91±2.87)min<(29.63±3.02)min]、呼吸恢复时间[(17.41±2.85)min<(23.67±3.49)min]相对更短,差异有统计学意义(χ^2=4.019,t=9.649、11.175、9.625,P<0.05)。结论老年患者四肢手术治疗期间,使用咪达唑仑与右美托咪定作为麻醉药物,能够确保手术安全、顺利进行,其中右美托咪定在镇静效果、安全性方面有着更为显著的优势。Objective To investigate the application effect of midazolam and dexmedetomidine in limb surgery of elderly patients,and evaluate the sedative effect and adverse reactions of patients.Methods A total of 96 patients who underwent limb surgery in the hospital from April 2018 to August 2019 were conveniently selected as the research objects,applied single blind random method,and they were divided into observation group(dexmedetomidine)and control group(midazolam),48 cases in each group,compared the sedative effects of the two groups.Results Compared with the control group,the excellent and good rate of sedation in the observation group(95.83%>77.08%)was relatively,the difference was statistically significant(χ^2=7.207,P<0.05),and the incidence of adverse reactions(4.16%<16.66%),cough times[(0.15±0.07)times/min<(0.32±0.10)times/min]relatively lower,recovery time[(22.91±2.87)min<(29.63±3.02)min],breathing recovery time[(17.41±2.85)min<(23.67±3.49)min]relatively shorter,the difference was statistically significant(χ^2=4.019,t=9.649,11.175,9.625,P<0.05).Conclusion During the surgical treatment of the limbs of elderly patients,midazolam and dexmedetomidine can be used as anesthetics to ensure the safe and smooth operation of the operation.Among them,dexmedetomidine has more sedative effects and safety,with significant advantage.
