摘　　要：目的以两种不同的评价标准探讨某院老年住院患者潜在不当用药(PIM)和潜在性处方遗漏(PPO)情况,为规范老年人临床用药提供参考。方法以Beers标准(2019版)及STOPP/START标准(2014版)为评判依据,回顾性分析该院2019年7月~9月的老年住院患者PIM和PPO情况。结果在364例患者中,Beers标准检出103例(28.3%)患者,涉及128项PIM,与专科相关的PIM 49项;STOPP/START标准检出90例(24.3%)患者,涉及98项PIM,其中与专科相关的PIM 37项;有106例(29.1%)患者存在138项PPO,其中与专科相关的PPO 53项。结论老年人的治疗过程中存在PIM和PPO,专科化的医师和药师在处理非专科疾病时可能出现PIM和PPO,通过一系列的改进可在一定程度上减少老年患者在诊治过程中的PIM和PPO。Objective To investigate the potential improper drug use(PIM)and potential prescription omission(PPO)in elderly inpatients in a hospital by using two different evaluation criteria,so as to provide reference for the standardization of clinical medication in elderly patients.Methods Based on the Beers criteria(2019 edition)and STOPP/START criteria(2014 edition),a retrospective case study was conducted to analyze the PIM and PPO status of elderly inpatients discharged from the hospital from July to September 2019.Results The Beers criteria identified 128 PIM in 28.3%(103)of the patients,49 PIM related to specialized subject.The STOPP/START criteria identified 98 PIM in 24.3%(90)of the patients,37 PIM related to specialized subject,identified 138 PPO in 29.1%(106)of the patients,53 PPO related to specialized subject.Conclusion The PIM and PPO in elderly patients are universal,they may caused by the treatment of the non-specialist disease by the pecialist clinicians and clinical pharmacists in,the PIM and PPO in process of diagnosis and treatment of the elderly patients can been reduced through a series of improvements,inclouding continuous learning and update the knowledge related professional,consultation of the physicians and pharmacists,the change of clinical pharmacist training and work.
