右美托咪定复合七氟醚行麻醉维持对腺样体肥大患儿术中血流动力学的影响

摘　　要：目的研究右美托咪定复合七氟醚行麻醉维持应用于儿童腺样体肥大切除手术的效果。方法选取2018年6月~2020年5月收治的腺样体肥大患儿98例,均行腺样体切除术,其中49例采用七氟醚麻醉维持为对照组,49例采用右美托咪定复合七氟醚麻醉维持为复合组。比较两组麻醉前(T0)、气管插管时(T1)、术后即刻(T2)血流动力学指标[平均动脉压(MAP)、心率(HR)]、术后6 h躁动率、疼痛程度[面部表情量表法(FPS评分)]及补救镇痛药舒芬太尼用量。结果T1、T2时复合组MAP、HR小于对照组(P<0.05);复合组躁动率6.12%小于对照组20.41%(P<0.05);复合组FPS评分低于对照组(P<0.05);复合组补救镇痛药舒芬太尼用量小于对照组(P<0.05)。结论右美托咪定复合七氟醚行麻醉维持应用于儿童腺样体肥大切除手术能维持术中血流动力学稳定,减少术后躁动情况发生,减轻疼痛程度,降低补救镇痛药舒芬太尼用量。Objective To study the effect of anesthesia maintenance with dexmedetomidine combined with sevoflurane in children with adenoid hypertrophy.Methods A total of 98 children with adenoid hypertrophy(June 2018 to May 2020)in our hospital were selected for adenoidectomy.Among them,49 cases were maintained with sevoflurane anesthesia as control group,and 49 cases were anesthetized with dexmedetomidine combined with sevoflurane and maintained in the composite group.The hemodynamics[mean arterial pressure(MAP),heart rate(HR)]before anesthesia(T0),during tracheal intubation(T1),immediately after surgery(T2),the agitation rate and pain level[facial expression scale method(FPS score)]at 6 h after operation and the amount of analgesic remedy were compared between the two groups.Results MAP and HR of the composite group at T1 and T2 were less than those of the control group(P<0.05);the agitation rate of 6.12%in the composite group was less than 20.41%in the control group(P<0.05);the FPS score in the composite group was lower than that in the control group(P<0.05);the amount of analgesic remedy for the composite group was less than that of the control group(P<0.05).Conclusion Dexmedetomidine combined with sevoflurane for anesthesia maintenance applied to adenoid hypertrophy can maintain intraoperative hemodynamic stability,reduce postoperative restlessness,and reduce the degree of pain and reduce the amount of analgesic remedy.
