摘 要:目的:对比分析腮腺多形腺瘤与腺淋巴瘤的MSCT征象,提高术前诊断准确性。方法:搜集31例(31个)腮腺多形腺瘤及17例(28个)腺淋巴瘤患者的病例资料,比较两组中患者的年龄、性别及病灶的位置、数目、最大径、密度及血管贴边征等征象。将病灶进一步分为密度均匀组和不均匀组、伴血管贴边征组和不伴血管贴边征组、肿瘤最大径>20 mm组和≤20 mm组,分析肿瘤密度和血管贴边征与肿瘤大小的关系。结果:两组间患者性别、年龄及病灶的位置、数目的差异有统计学意义(P<0.05),而肿瘤最大径的差异无统计学意义(P>0.05)。多形性腺瘤中密度均匀组和不均匀组的肿瘤最大径分别为(22.60±6.54)和(27.15±11.03)mm,两组间的差异无统计学意义(P>0.05);腺淋巴瘤中密度均匀组与不均匀组的肿瘤最大径分别为(18.92±9.45)和(26.87±8.10)mm,两组间差异有统计学意义(P<0.05)。肿瘤最大径≤20 mm组(23例)中,多形性腺瘤10个,包括密度均匀2个、密度不均匀8个,腺淋巴瘤13个,包括密度均匀11个、密度不均匀2个,两者间差异有统计学意义(P<0.05);而在肿瘤最大径>20 mm组(25例)中,多形性腺瘤21个,包括密度均匀3个、密度不均匀18个,腺淋巴瘤15个,包括密度均匀2个、密度不均匀13个,二者间差异无统计学意义(P>0.05)。多形性腺瘤中伴血管贴边征组与不伴血管贴边征组的肿瘤最大径分别为(29.55±10.45)和(20.73±8.20)mm,两组间差异有统计学意义(P<0.05);腺淋巴瘤中伴血管贴边征组与不伴血管贴边征组的肿瘤最大径分别为(24.48±9.65)和(17.20±6.38)mm,两组间差异无统计学意义(P>0.05)。结论:在最大径≤20 mm的腮腺肿瘤中,密度是否均匀及血管贴边征对鉴别多形性腺瘤与腺淋巴瘤具有重要价值,但在最大径>20 mm的腮腺肿瘤中这2个征象的鉴别诊断价值不大。Objective:To compare the multi-slice CT features of pleomorphic adenoma and adenolymphoma of parotid gland,thus to improve the preoperatively diagnosis accuracy.Methods:59 lesions from 31 cases with pleomorphic adenoma(n=31)and 17 cases with adenolymphoma(n=28)were enrolled in this study.The clinical data(age,sex),and MSCT features(location of the lesions,number of lesions,tumor maximum diameter,density and tumor Vaso-welt sign)of pleomorphic adenoma and adenolymphoma were analyzed and compared.All cases were further divided into subgroups according to uniform and non-uniform density,with or without Vaso-welt sign,and maximum diameter>20mm and≤20mm.The relationship of tumor density,Vaso-welt sign with tumor size was analyzed.Results:The gender,age,location and density of the lesions showed statistically significant diffe-rence between pleomorphic adenoma and adenolymphoma(all P<0.05);there was no statistically significant difference(P>0.05)in the maximum diameter between pleomorphic adenoma and adenolymphoma.In the pleomorphic adenoma group,there was no significant difference(P>0.05)in the maximum diameter between the uniform density group and the uneven density group[(22.60±6.54)vs.(27.15±11.03)mm];in the adenolymphoma group,there was statistically significant difference(P<0.05)in the maximum diameter between the uniform density group and the uneven density group[(18.92±9.45)vs.(26.87±8.10)mm].In the group of the largest tumor diameter≤20mm,there were 10 pleomorphic adenomas with uniform density(n=2)or uneven density(n=8),and 13 adenomas with uniform density(n=11)or uneven density(n=2),the difference was statistically significant(P<0.05);In the group of the largest tumor diameter>20mm,there were 21 pleomorphic adenomas with uniform density(n=3)or uneven density(n=18),and 15 adenoma with uniform density(n=2)or uneven density(n=13),there was no significant difference between the two types(P>0.05).In pleomorphic adenomas,the difference in maximum diameter[(29.55±10.45)vs.(20.73±8.20)mm]between the group w
