摘　　要：目的探讨血清Presepsin对早产儿早发型脓毒症的诊断价值。方法选取医院2017年6月至2018年12月收治的早产儿103例,按是否为早发型脓毒症分为EOS组(47例)和对照组(56例)。在入院后0 h(T0)、24 h(T24)、48 h(T48)采集早产儿静脉血,检测血清Presepsin、降钙素原(PCT)和C反应蛋白(CRP)表达水平,判断上述指标的诊断价值。结果T24时,Presepsin,PCT,CRP表达水平均显著升高(P<0.01);AUC分别为0.978[95%CI(0.939,1.00)]、0.851[95%CI(0.730,0.925)]和0.761[95%CI(0.699,0.871)],最佳诊断界值分别为637 ng/L、8.4 ng/mL,20.5 mg/L,诊断灵敏度和诊断特异度分别为91%和100%,87%和84%,68%和89%。结论Presepsin可作为早产儿早发型败血症早期诊断的重要指标。Objective To investigate the diagnostic value of serum Presepsin in premature infants with early-onset sepsis.Methods A total of 103 premature infants admitted to our hospital from June 2017 to December 2018 were selected and divided into EOS group(47 cases)and control group(56 cases)according to whether they were early-onset sepsis.Venous blood samples were collected at 0 h(T0),24 h(T24)and 48 h(T48)after admission.The expression levels of serum presepsin,procalcitonin(PCT)and C-reactive protein(CRP)were detected to determine the diagnostic value of the above indicators.Results At T24,the expression levels of presepsin,PCT and CRP were significantly increased(P<0.01).AUC were 0.978[95%CI(0.939,1.00)],0.851[95%CI(0.730,0.925)]and 0.761[95%CI(0.699,0.871)],respectively.The best diagnostic cutoff values were 637 ng/L,8.4 ng/mL,20.5 ng/mL.The sensitivity and specificity were 91%and 100%,87%and 84%,68%and 89%,respectively.Conclusion Presepsin can be used as an important index for early diagnosis of premature infants with earlyonset sepsis.
