摘  要:目的:观察淫羊藿甙干预的脂肪间充质干细胞(AMSCs)治疗兔骨性关节炎的效果,并分析其可能的作用机制。方法:取30只新西兰大白兔建立右后肢骨性关节炎模型,造模成功后随机分为3组,每组10只。对照组右后肢膝关节腔内注射同种异体AMSCs进行移植治疗,实验组注射淫羊藿甙溶液干预的同种异体AMSCs进行移植治疗,空白组注射等量的生理盐水。ELISA法检测3组膝关节液内NO、IL-1与TNF-α等炎症因子水平;观察3组关节软骨组织病理改变;TUNEL染色观察关节软骨内软骨细胞凋亡;Western blot检测3组软骨组织内TLR4、MyD88、TRAF6与NF-κB p65蛋白表达。结果:空白组NO、IL-1与TNF-α浓度升高,关节软骨退变、降解,软骨细胞大量凋亡,软骨组织内TLR4、MyD88、TRAF6与NF-κB p65蛋白表达增加;与空白组相比,对照组NO、IL-1与TNF-α浓度降低,关节软骨退变、降解减轻,软骨细胞凋亡减少,软骨组织内TLR4、MyD88、TRAF6与NF-κB p65蛋白表达减少;与对照组相比,实验组NO、IL-1与TNF-α浓度降低,关节软骨退变、降解减轻,软骨细胞凋亡减少,软骨组织内TLR4、MyD88、TRAF6与NF-κB p65蛋白表达减少。结论:淫羊藿甙可降低关节腔内NO、IL-1与TNF-α等炎症因子水平,改善骨性关节炎的微环境,抑制关节软骨细胞凋亡,从而达到保护关节软骨的作用,该作用可能与调节TLR4/NF-κB信号通路有关。Objective:To observe the effect of Icariin intervention on adipose mesenchymal stem cells(AMSCs)in the treatment of osteoarthritis in rabbits,and analyze its possible mechanism.Methods:30 New Zealand white rabbits were selected to establish the right hindlimb osteoarthritis model.After the model was successfully established,they were randomly divided into 3 groups,10 in each group.In control group,the right hindlimb knee joint was injected with allogeneic AMSCs for transplantation,in experimental group,the allogeneic AMSCs were injected with Icariin solution for transplantation,and in blank group,the same amount of normal saline was injected.Levels of NO,IL-1,TNF-α and other inflammatory factors in knee joint fluid of the 3 groups were measured by ELISA method;pathological changes of the articular cartilage of the 3 groups were observed;apoptosis of chondrocytes in the articular cartilage was observed by TUNEL staining;expression of TLR4,MyD88,TRAF6 and NF-κB p65 in cartilage of the 3 groups were detected by Western blot.Results:In blank group,concentration of NO,IL-1 and TNF-α increased,articular cartilage degenerated and degraded,chondrocytes apoptosis,TLR4,MyD88,TRAF6 and NF-κB p65 protein expression increased;compared with blank group,concentration of NO,IL-1 and TNF-α decreased,articular cartilage degeneration and degradation decreased;compared with control group,concentration of NO,IL-1 and TNF-α in experimental group decreased,degeneration and degradation of articular cartilage decreased,apoptosis of chondrocytes decreased,and expression of TLR4,MyD88,TRAF6 and NF-κB p65 in cartilage decreased.Conclusion:Icariin can reduce levels of NO,IL-1 and TNF-α in articular cavity,improve the microenvironment of osteoarthritis,inhibit apoptosis of articular chondrocytes,and protect articular cartilage,which may be related to the regulation of TLR4/NF-κB signaling pathway.
