摘 要:目的探究微小RNA-590(miR-590)在IgA肾病(IgAN)患者尿液及血清中的表达水平及其临床意义。方法选取2017年1月~2020年2月本院诊治的IgAN患者85例为IgAN组,并选取同期体检健康者87例为对照组。实时荧光定量PCR(qRT-PCR)法检查两组尿液、血清miR-590表达水平;采用酶联免疫吸附法(ELISA)检测两组尿液、血清血管抑素水平;分析比较不同程度IgAN患者尿液、血清miR-590、血管抑素水平;采用Pearson法分析IgAN患者尿液、血清miR-590表达水平与血管抑素的关系;采用受试者工作特征曲线(ROC)评价尿液及血清中miR-590、血管抑素对IgAN的诊断价值。结果IgAN组患者尿液、血清中miR-590、血管抑素水平均明显高于对照组(P<0.05);中度组、重度组IgAN患者尿液miR-590、血管抑素水平均显著高于轻度组(P<0.05),且两者水平随疾病程度加重而明显升高;IgAN患者尿液、血清miR-590表达水平均与血管抑素水平均呈正相关(P<0.05);尿液、血清miR-590诊断IgAN的曲线下面积(AUC)分别为0.837、0.822,截断值分别为1.36、1.38,此时对应灵敏度分别为76.5%、72.9%,特异性为80.5%、80.5%;尿液、血清血管抑素诊断IgAN的AUC分别为0.849、0.833,截断值分别为59.25ng/L,60.58ng/L,此时对应灵敏度分别为78.8%、71.8%,特异性为88.5%、87.4%;miR-590、血管抑素分别在尿液、血清中联合诊断IgAN的AUC分别为0.903、0.910,灵敏度分别为82.4%、91.8%,对应特异度分别为86.2%、80.5%。结论IgAN患者尿液、血清miR-590呈高表达,其在尿液及血清中均与血管抑素有较强正相关性,两者联合可有效提高对IgAN的诊断价值。Objective To investigate the expression levels of microRNA-590(miR-590)in urine and serum of patients with IgA nephropathy(IgAN)and their clinical significance.Methods From January 2017 to February 2020,85 patients with IgAN in our hospital were selected as the IgAN group,and 87 healthy people in the same period were selected as the control group.The expression of miR-590 in urine and serum of the two groups was detected by real-time fluorescent quantitative PCR(qRT-PCR).The levels of angiostatin in urine and serum were detected by enzyme-linked immunosorbent assay(ELISA).The levels of miR-590 and angiostatin in urine and serum of patients with different levels of IgAN were compared.Pearson’s method was used to analyze the relationships between the expressions of miR-590 and angiostatin in urine and serum.The diagnostic value of miR-590 and angiostatin in urine and serum for IgAN was evaluated by receiver operating characteristic curve(ROC).Results The levels of miR 590 and angiostatin in urine and serum of IgAN group were significantly higher than those of control group(P<0.05).The levels of miR 590 and angiostatin in urine of IgAN patients in moderate and severe groups were significantly higher than those in mild group(P<0.05).The expression of miR 590 in urine and serum was positively correlated with angiostatin level in IgAN patients(P<0.05).The area under the curve(AUC)of urine and serum miR 590 were 0.837 and 0.822,respectively.The cut-off values were 1.36 and 1.38,respectively.The corresponding sensitivities were 765%and 729%,respectively,and the specificities were 80.5%and 80.5%,respectively.The AUC of urine and serum angiostatin in the diagnosis of IgAN were 0.849 and 0.833,respectively.The cut-off values were 59.25ng/L and 60.58ng/L,respectively.The corresponding sensitivities were 78.8%and 71.8%,respectively,and the specificities were 885%and 874%,respectively.The AUC of miR 590 and angiostatin in urine and serum were 0.903 and 0.910 respectively.The sensitivity was 82.4%and 91.8%respectively.The spe
