连续胰岛素皮下输注强化治疗糖尿病合并肺结核的临床分析

摘　　要：目的分析连续胰岛素皮下输注强化治疗糖尿病合并肺结核的临床疗效。方法选取该院在2017年3月—2019年8月收治的62例糖尿病合并肺结核患者为试验样本,分组以随机抽签的方式进行,对照组31例患者采取常规皮下注射胰岛素治疗,试验组31例患者采取连续胰岛素皮下输注治疗,就其疗效进行比较。结果治疗后试验组患者FBG、2 hPBG、HbAlc水平均显著低于对照组,痰菌转阴率(77.4%)显著高于对照组(45.2%),血糖达标时间明显短于对照组,每天胰岛素用量显著少于对照组,差异有统计学意义(P<0.05)。试验组患者肺结核病灶吸收有效率和空洞闭合率均有好转,差异无统计学意义(P>0.05)。结论对糖尿病合并肺结核患者应用连续胰岛素皮下输注强化治疗可取得很好的临床疗效,可有效改善其血糖水平,减少胰岛素用量,并改善肺结核病情,建议将此治疗方案在临床中推广应用。Objective To analyze the clinical efficacy of continuous subcutaneous insulin infusion for intensive treatment of diabetes mellitus complicated with tuberculosis.Methods The 62 patients with diabetes and tuberculosis admitted to the hospital from March 2017 to August 2019 were selected as the test samples,and the grouping was carried out in a random lottery manner.31 patients in the control group received conventional subcutaneous insulin treatment,and 31 patients in the test group Take continuous subcutaneous insulin infusion to compare its efficacy.Results After treatmene the levels of FBG,2 hPBG,and HbAlc in the test group were significantly lower than those in the control group.The negative rate of sputum bacteria(77.4%)was significantly higher than that of the control group(45.2%).The blood glucose reaching standard time was significantly shorter than that of the control group,the difference was statistically significant(P<0.05).In the experimental group,the effective rate of pulmonary tuberculosis lesion absorption and cavity closure rate improved,the difference was not statistically significant(P>0.05).Conclusion The continuous subcutaneous insulin infusion intensive treatment for diabetic patients with pulmonary tuberculosis can achieve good clinical effects,which can effectively improve the blood glucose level,reduce the amount of insulin,and improve the condition of pulmonary tuberculosis.It is recommended that this treatment plan be promoted and applied in the clinic.
