从李中梓治泻九法谈温补脾肾在慢性溃疡性结肠炎治疗中的应用

摘　　要：慢性溃疡性结肠炎(UC)属于中医学"肠澼""下利""痢疾"等范畴,临床可表现为久泻夹带黏液脓血、腹痛、形寒肢冷等脾肾阳虚、风湿阻滞证,慢性UC与"泄泻"临床表现相似,风湿寒热四气皆可致泄,为慢性UC发病之标;四邪中湿邪为泄泻的主要病因,风、寒、热三邪皆可通过五行生克关系与湿邪相联系,影响慢性UC的病情转变,先后天失调所致脾肾阳虚为慢性UC发病之本。著名温补医家李中梓重视温补脾肾,针对泄泻,创立治泻九法。李中梓认为一切虚证归属脾肾,脾肾阳虚,风湿阻滞为慢性UC的根本。李中梓治泻九法广泛应用于UC的各个发病阶段,其中升提法、燥脾法和温肾法在慢性期最为多见。Chronic ulcerative colitis(UC)belongs to the category of Chang pi,Xia li and Li ji in traditional Chinese medicine,which clinically manifests as syndrome of yang deficiency of spleen and kidney and wind-dampness stagnation with chronic diarrhea with mucus and pus,abdominal pain,cold in body and limbs.The clinical manifestations of chronic UC are similar to Xie xie(diarrhea).Wind,dampness,cold and heat can cause diarrhea which is the symptoms of chronic UC.Dampness is the main cause of diarrhea while the other three pathogenic factors can be associated with dampness through the relationship of five elements,which can affect the change of chronic UC.Yang deficiency of spleen and kidney by congenital and acquired disorders is the root cause of chronic UC.Li zhong-zi,a famous Chinese doctor,attached great importance to warming and tonifying the spleen and kidney.Aiming at diarrhea,he established nine methods to treat diarrhea.Li zhong-zi thought that all deficiency syndromes belong to spleen and kidney,and spleen-kidney yang deficiency,wind-dampness stagnation is the root of chronic UC.Li zhong-zi's nine methods are widely used in various stages of UC,among which ascending and extracting method,drying spleen method and warming kidney method are the most common in the chronic stage.
