抗核抗体和抗核抗体谱联合检测诊断自身免疫性疾病的临床价值

摘　　要：目的探讨应用抗核抗体与抗核抗体谱联合检测对于诊断自身免疫性疾病的临床价值。方法该院方便选取2017年1月—2019年12月期间收治的最终确诊为自身免疫性疾病的97例患者进行研究,分别采用酶联免疫吸附测定法(ELISA)和线性免疫印迹法对所有患者的抗核抗体(ANA)和抗核抗体谱(ANAs)进行检测,并进行统计分析。结果女性患者检出各项的阳性率:ANA(75.81%)、Ul-SnRNP(8.06%)、抗SSA(37.10%)、抗SSB(20.97%)、抗核小体(30.65%)和抗组蛋白(37.10%),明显高于男性患者,均差异有统计学意义(χ^2=6.014、4.273、2.042、2.026、2.021、4.250,P<0.05)。抗SmDl、抗Jo-1、抗Scl-70在性别间比较,均差异无统计学意义(P>0.05)。经不同检测方法与AID确诊情况对比,结果发现ANA检测的准确率为67.01%,ANAs检测的准确率为92.78%。ANAs检测与ANA检测结果对比,差异有统计学意义(χ^2=20.058,P<0.05)。结论采用抗核抗体谱联合检测对自身免疫性疾病进行诊断,相比于单纯应用抗核抗体检测能够有效提高该病的诊断率,降低失误,对于自身免疫性疾病(AID)的诊断和治疗具有重要价值。Objective To explore the clinical value of combined detection of antinuclear antibody and antinuclear antibody profile for the diagnosis of autoimmune diseases.Methods The hospital convenient selected 97 patients who were finally diagnosed as autoimmune diseases admitted from January 2017 to December 2019 for research.All patients were treated by enzyme-linked immunosorbent assay(ELISA)and linear western blotting respectively.The anti-nuclear antibody(ANA)and anti-nuclear antibody profile(ANAs)were tested and statistically analyzed.Results ANA(75.81%),Ul-SnRNP(8.06%),anti-SSA(37.10%),anti-SSB(20.97%),anti-nucleosome(30.65%)and anti-histone(37.10%)were detected in female patients,whose positivity rate was higher than that of male patients,the difference was statistically significant(χ2=6.014,4.273,2.042,2.026,2.021,4.250,P<0.05).There was no statistically significant difference in anti-SmDl,anti-Jo-1,and anti-Scl-70 between sexes(P>0.05).After comparing different detection methods with the diagnosis of AID,it was found that the accuracy of ANA detection was 67.07%,and the accuracy of ANAs detection was 92.78%.The comparison between ANAs test and ANA test results was statistically significant difference(χ2=20.058,P<0.05).Conclusion The combined detection of antinuclear antibody spectrum for the diagnosis of autoimmune diseases can effectively improve the diagnosis rate of the disease,reduce errors,and diagnose and treat autoimmune diseases(AID)compared to purely using antinuclear antibody detection,it has important value.
