骨创伤患者血浆D-二聚体和血糖水平变化及临床参考价值探析

摘　　要：目的分析骨创伤患者创伤后的血浆D-二聚体(D-D)与血糖水平的变化情况,探讨其与创伤之间的相关性和临床意义。方法方便选择该院2018年1—12月住院的45例骨创伤患者,另外选择同时在该院门诊接受体检的45名健康人群作为对照。分别动态测定观察组和对照组在血浆D-D和血糖水平指标方面的差异,分析其临床参考价值。结果创伤患者血浆D-D指标高于健康者的(181.5±22.1)μg/L,血糖值高于健康者的(5.9±0.8)mmol/L,差异有统计学意义(t=3.892、3.771,P<0.05),且随着创伤程度的增加,各项指标也同步增加,证实了骨创伤患者创伤后的指标检测具有关键作用。结论患者出现骨创伤后机体出现的应激反应会影响到血浆D-D和血糖水平指标,可以将其判定为创伤严重程度的参考依据,从而确定患者实际情况,作出预后治疗判断。Objective To analyze the changes of plasma D-dimer(D-D)and blood glucose levels after trauma in patients with bone trauma,and to understand the correlation and clinical significance between these indicators and trauma.Methods The study subjects conveniently selected 45 patients with bone trauma who were hospitalized in the hospital from January to December 2018,and 45 healthy people who received physical examinations at the outpatient clinic of our hospital were selected as controls.The differences in plasma D-D and blood glucose level indexes of the combined control group were dynamically determined and observed respectively,and their clinical reference value was analyzed.Results The plasma DD index of trauma patients was higher than that of healthy people(181.5±22.1)μg/L,and the blood glucose level is higher than that of healthy people(5.9±0.8)mmol/L,and the difference in the results of the study was statistically significant(t=3.892,3.771,P<0.05),and as the degree of trauma increases,the indicators also increase,confirming that the post-traumatic index detection of bone trauma patients had a key role.Conclusion The body's stress response after bone trauma can affect the plasma D-D and blood glucose level indicators,which can be judged as a reference basis for the severity of trauma,so as to determine the actual situation of the patient and make prognostic treatment judgments.
