右半结肠肿瘤伴梗阻手术一期吻合可行性分析

摘　　要：目的分析右半结肠肿瘤伴梗阻手术一期吻合可行性,结合患者的术后恢复情况进行评价。方法方便选取该院2012年1月—2019年4月期间收治的93例右半结肠肿瘤伴梗阻患者作为研究对象,应用随机单盲法,分为观察组(一期肿瘤切除加吻合加末段回肠双攀造口)50例和对照组(肿瘤切除加末段回肠造口)43例,观察两组患者的术后恢复效果。结果观察组患者的术后首次排气时间(2.61±0.54)d、术后引流管拔出时间(5.36±0.83)d、住院时间(12.97±1.95)d相比对照组更短,差异有统计学意义(t=13.796、9.798、6.815,P<0.05),并发症发生率4.00%,相比对照组20.93%更低,差异有统计学意义(χ^2=6.354,P<0.05),预后优良率94.00%,相对对照组76.74%更高,差异有统计学意义(χ^2=5.725,P<0.05)。结论右半结肠肿瘤伴梗阻患者接受手术治疗期间,一期肿瘤切除加吻合加末段回肠双攀造口是可行性的治疗操作,有助于患者术后的快速、良好恢复,减少并发症的发生。Objective To analyze the feasibility of one-stage anastomosis of right colon tumor with obstruction,and evaluate the patient's postoperative recovery.Methods Ninety-three patients with right colon tumors with obstruction admitted in the hospital from January 2012 to April 2019 were selected as the research objects,and they were divided into 50 cases of the observation group(one-stage tumor resection plus anastomosis plus double climbing stom of the terminal ileum and 43 cases of the control group(tumor resection plus terminal ileostomy).Observed the postoperative recovery effect of the two groups.Results Compared with the control group,the first postoperative exhaust time(2.61±0.54)d,the postoperative drainage tube withdrawal time(5.36±0.83)d,hospitalization time(12.97±1.95)d of the observation group were relatively shorter,the difference was statistically significant(t=13.796,9.798,6.815,P<0.05),complication rate of observation group 4.00%relatively lower than the control group 20.93%,the difference was statistically significant(χ2=6.354,P<0.05),and the prognostic good rate of observation group 94.00%relatively higher than the control group 76.74%,the difference was statistically significant(χ2=5.725,P<0.05).Conclusion During the surgical treatment of patients with right colon tumor and obstruction,one-stage tumor resection plus anastomosis plus terminal ileum double climbing stoma is a feasible treatment operation,which will help the patient to recover quickly and well after surgery and reduce the occurrence of complications.
