利用PASS系统回顾性分析门诊处方剂量范围的潜在风险

摘　　要：目的促进临床遵循药品说明书合理用药,降低门诊处方剂量范围的潜在风险。方法根据PASS系统对门诊处方的警示类型统计结果,主要潜在风险是剂量范围,警示信息46692条(59.49%),以前40位药品的警示信息(共44590条)作为分析对象,以药品说明书和2015年版《中华人民共和国药典临床用药须知》的用法用量作为剂量范围合理性的判定标准。结果该院门诊处方的剂量范围潜在风险警示,其中用药频次占67.75%,用药剂量占15.57%,其他问题占16.68%。结论PASS系统对门诊处方的潜在风险有警示价值,但需加强系统的日常维护,提高警示信息的准确性;利用PASS系统可对用药潜在风险进行统计和分析,将结果作为药事管理的重要内容,可以对临床提出二次警示,降低患者的用药潜在风险。Objective To promote the rational use of drugs according to the drug instructions and reduce the potential risk of outpatient prescription dose range.Methods According to the statistical results of the warning types of outpatient prescriptions in pass system,the main potential risk was the dose range,with 46692 pieces of warning information(59.49%),and 44590 pieces of warning information of the first 40 drugs as the analysis object.The usage and dosage of the drug instructions and the 2015 edition of The Clinical Medication Instructions of China Pharmacopoeia were used as the judgment standard of the rationality of the dose range.Results The potential risk warning of the dose range of outpatient prescriptions in our hospital,in which the frequency of drug use accounted for 67.75%,the dose of drug use accounted for 15.57%,and other problems accounted for 16.68%.Conclusion Pass system has warning value for potential risks of outpatient prescriptions,but it needs to strengthen the daily maintenance of the system and improve the accuracy of warning information;the use of pass system can make statistics and analysis on potential risks of medication,and take the results as an important part of pharmaceutical management,which can put forward secondary warning to the clinic and reduce the potential risks of patients.
