分期电针联合头穴久留针治疗对急性缺血性脑卒中患者下肢运动功能的影响

摘　　要：目的:观察分期电针联合头穴久留针治疗对急性缺血性脑卒中患者下肢运动功能的影响。方法:采用随机数表法将本院2016年12月至2019年12月收治的84例急性缺血性脑卒中患者分为对照组和观察组,各42例。对照组行常规针刺治疗,观察组行分期电针联合头穴久留针辅助治疗,采用简式Fugl-Meyer运动功能量表评定患者治疗期间肢体运动情况,足印法收集步态资料评价其步行能力,改良Barthel指数评定量表(MBI)和SF-36对比两组患者的生活质量。结果:治疗后,两组的Fugl-Meyer均呈逐步升高趋势(P<0.05),且观察组治疗4周、治疗后评分均明显高于对照组(P<0.05)。治疗后两组的患侧跨步长增加,患侧与健侧步长差缩小,步频加快,最大步行速度增加(P<0.05),且观察组明显优于对照组(P<0.05)。治疗后,两组MBI评分均呈逐步升高趋势(P<0.05),且观察组治疗4周、治疗后的评分均明显高于对照组(P<0.05)。观察组SF-36生活质量各指标评分均低于对照组(P<0.05)结论:分期电针联合头穴久留针治疗急性缺血性脑卒中患者,可明显改善其下肢运动功能,提高其步行能力和生活能力。Objective: To investigate the effect of staged electroacupuncture combined with long-term scalp acupuncture on the lower limb motor function for patients with acute ischemic stroke. Methods: 84 patients with acute ischemic stroke admitted to the hospital from December 2016 to December 2019 were divided into a controlled group and an observation group by the random number table method, 42 cases in each group. The controlled group were treated with conventional acupuncture, and the observation group were treated with staged electroacupuncture combined with long-term scalp acupuncture. The simple Fugl-Meyer motor function scale was used to evaluate the patient’s limb movement during treatment. The gait data were collected by foot-print to evaluate walking ability. The modified Barthel Index(MBI) and SF-36 were used to evaluate quality of life. Results: After treatment, the Fugl-Meyer scores of both groups showed an increasing trend(P<0.05). Besides, the scores of the observation group after 4 weeks of treatment and after treatment were significantly higher than those of the controlled group(P<0.05). After treatment, the stride length of affected sides of both groups increased, and the difference of step length between the affected side and the healthy side decreased. The stride frequency increased, and the maximum walking speed increased(P<0.05). Besides, the above indexes in the observation group were significantly better than those in the controlled group(P<0.05). After treatment, the MBI scores of both groups showed a upward trend(P<0.05),and the scores of the observation group after 4 weeks of treatment and after treatment were significantly higher than the controlled group(P<0.05). The SF-36 quality of life scores of the observation group were lower than those of the controlled group(P<0.05).
