低频rTMS联合电针风府哑门治疗脑卒中后Broca失语临床研究

摘　　要：目的:观察低频重复经颅磁刺激(rTMS)联合电针风府哑门治疗脑卒中后Broca失语的临床疗效。方法:80例脑卒中后Broca失语患者随机分为观察组和对照组各40例。观察组采用低频rTMS联合电针风府哑门治疗,对照组采用低频rTMS治疗,每天1次,每周5次,共4周1疗程,比较两组治疗前后语言功能评分及临床疗效差异。语言功能评分采用汉语失语成套检测(ABC)及失语商(AQ),临床疗效评定采用波士顿失语诊断测验(BDAE)。结果:观察组总有效率为82.5%,高于对照组的70.0%(P<0.05)。与治疗前比较,两组患者ABC(言语、理解、复述、命名)评分、AQ及BDAE评分均升高(P<0.05),观察组升高优于对照组(P<0.05)。结论:低频rTMS联合电针风府哑门治疗脑卒中后Broca失语临床疗效显著,有利于改善患者语言功能,促进患者早日回归家庭社会。Objective: To observe the clinical effect of applying low frequency repetitive transcranial magnetic stimulation(rTMS) combined with electroacupuncture on Fengfu and Yamen in the treatment of Broca aphasia after stroke. Methods: 80 patients with post-stroke Broca aphasia were randomly divided into an observation group and a controlled group, 40 cases in each group. The observation group was treated with low-frequency rTMS combined with electroacupuncture at Fengfu and Yamen. The controlled group was treated with low-frequency rTMS once a day, five times a week, a total of 4 weeks. The differences of language function score and clinical effect between the two groups before and after treatment were compared. Chinese aphasia test(ABC) and aphasia quotient(AQ) were used for language function score, and Boston aphasia diagnostic test(BDAE) was used for clinical efficacy evaluation. Results: The total effective rate of the observation group was 82.5%,higher than 70.0% of the controlled group(P<0.05). Compared with before treatment, the scores of ABC(speech, understanding, retelling, naming),AQ and BDAE in the two groups were all increased(P<0.05),and the increase in the observation group was better than that in the controlled group(P<0.05). Conclusion: Low frequency rTMS combined with electroacupuncture at Fengfu and Yamen is effective in the treatment of Broca aphasia after stroke, which is beneficial to improve the language function of patients and promote the early return of patients to family and society.
