“耳胀”“耳闭”病名考证

摘　　要：耳胀耳闭是以耳内胀闷堵塞感及听力下降为主要症状的中耳疾病,教材和相关指南认为西医学的分泌性中耳炎属于此范畴。“耳胀耳闭”是以2个名词排列形成的一个疾病名称,因此并不符合疾病的命名原则。为寻找更为规范的命名,本文通过梳理古现代文献中关于“耳胀”“耳闭”的记载,发现“耳胀”“耳闭”作为名词在古代文献中分开记载,多被描述为疾病的症状。此外,与“耳胀”“耳闭”描述相似甚或相同的疾病病名有“风聋”“暴聋”“卒聋”“气闭耳聋”以及现代的“急、慢耳痹”。通过考证,“风聋”“暴聋”“卒聋”“气闭耳聋”均不能够取代“耳胀耳闭”“急、慢耳痹”为新命名疾病且影响面较小有待进一步考辨,另期待广大同仁能够寻找更为合适的病名。The ear fullness and block is a kind of middle ear disease characterized by expansion feeling,clogging or even hearing loss,which is considered in textbooks and related guidelines as a kind of secretory otitis media in western medicine."The ear fullness and block"is a disease name formed by the arrangement of two nouns,so it does not conform to the naming principle of the disease.In order to find a more standardized name,this paper,by combing the ancient and modern literature about:fural fullness"and"blocked ear"records,found that"anemogenous deafness""sudden deafness""fulminant deafness""deafness with Qi stagnation"and modern"acute,slow ear obstruction"are similar to or even the same as the description of"fural fullness"and"blocked ear".Through textual research,"anemogenous deafness""sudden deafness""fulminant deafness""deafness with Qi stagnation"can not replace"the ear fullness and block".The influence of"acute,slow ear obstruction"is small,which needs to be further studied.In addition,it is expected that the majority of colleagues can find a more suitable name of the disease.
