分娩监护镇痛系统和硬膜外麻醉在足月初产妇分娩过程中的应用效果比较

摘　　要：目的:比较分娩监护镇痛系统和硬膜外麻醉在足月初产妇分娩过程中的临床效果。方法:选取本院2018年12月-2019年12月产科分娩的足月初产妇100例为研究对象,随机分成镇痛系统组(给予分娩监护镇痛系统进行分娩镇痛)和硬膜外麻醉组(给予硬膜外麻醉方式进行分娩镇痛),各50例。对两组的疼痛情况、C反应蛋白和皮质醇水平及各产程时间进行观察和比较。结果:镇痛10 min后,两组VAS评分均得到显著改善,均低于镇痛前,差异均有统计学意义(P<0.05);镇痛10 min后,两组产妇VAS评分比较差异无统计学意义(P>0.05);产后24 h,镇痛系统组C反应蛋白和皮质醇水平均优于硬膜外麻醉组,差异均有统计学意义(P<0.05);镇痛系统组第一产程和第二产程时间均短于硬膜外麻醉组,差异均有统计学意义(P<0.05)。结论:分娩监护镇痛系统应用于足月初产妇分娩过程中镇痛效果良好,并能够改善产妇应激状态,有效缩短产程。Objective:To compare the clinical effect of labor monitoring and analgesia system and epidural anesthesia in the delivery of primipara with full term pregnancies.Method:From December 2018 to December 2019,a total of 100 primiparas with full term pregnancies in our hospital were randomly divided into the analgesia system group (giving labor monitoring and analgesia system for labor analgesia) and the epidural anesthesia group (giving epidural anesthesia for labor analgesia),50 cases in each group.Between the two groups,the pain,level of C-reactive protein,level of cortisol and the time of labor were observed and compared.Result:After 10 min of analgesia,the VAS scores of the two groups were significantly improved,which were lower than before analgesia (P<0.05).After 10 min of analgesia,there was no statistically significant difference in VAS scores between the two groups (P>0.05).After 24 h postnatal,the levels of C-reactive protein and cortisol of the analgesia system group were better than those of the epidural anesthesia group (P<0.05).The time of the first and second stages of labor in the analgesia system group was shorter than that in the epidural anesthesia group (P<0.05).Conclusion:The labor monitoring and analgesia system have a good analgesic effect in the delivery of primipara with full term pregnancies,it can improve the stress state of parturient and shorten the labor process.
