重组人碱性成纤维细胞生长因子联合VSD对美容缝合伤口愈合质量的临床研究

摘　　要：目的:观察重组人碱性成纤维细胞生长因子联合封闭负压引流(vacuum-sealing drainage,VSD)对美容缝合伤口愈合质量的效果。方法:选取本院2016年1月-2019年3月92例躯干体表包块行切除手术患者,所有患者均行美容缝合,按随机数字表方法分为生长因子组30例、VSD组31例、联合组31例。比较三组住院换药次数、术后14 d伤口愈合情况、术后12个月患者满意度、视觉模拟量表(VAS),患者瘢痕评估量表(patient scar assessment scale,PSAS)及温哥华瘢痕量表(Vancouver scar scale,VSS)。结果:三组伤口愈合情况比较差异无统计学意义(P>0.05),生长因子组和VSD组换药次数、患者满意度、VAS、PSAS、VSS评分比较差异无统计学意义(P>0.05),联合组换药次数、患者满意度、VAS、PSAS、VSS评分均优于生长因子组和VSD组,差异均有统计学意义(P<0.05)。结论:对于美容缝合的患者,重组人碱性成纤维细胞生长因子联合VSD能够有效降低患者换药次数,提高患者满意度,并降低瘢痕面积,值得临床上开展应用。Objective:To observe the effect of recombinant human basic fibroblast growth factor combined with vacuum-sealing drainage (VSD) on the quality of cosmetic suture wound healing.Method:Ninety cases of patients underwent cosmetic suture after resection of the trunk surface mass in our hospital from January 2016 to March 2019 were selected.They were randomly divided into the growth factor group (n=30),the VSD group (n=31) and the combination group (n=31).The times of dressing change,wound healing at 14 d after operation,patient satisfaction and visual analogue scale (VAS) and patient scar assessment scale (PSAS) and Vancouver scar scale (VSS) scores were compared among the three groups at 12 months after operation.Result:There were no statistical differences in wound healing among the three groups (P>0.05).There were no significant differences in times of dressing change,patient satisfaction,VAS,PSAS and VSS scores between the growth factor group and the VSD group (P>0.05).The times of dressing change,patient satisfaction,VAS,PSAS and VSS scores of the combined group were better than those of the growth factor group and the VSD group,the differences were statistically significant (P<0.05).Conclusion:Recombinant human basic fibroblast growth factor combined with VSD can effectively reduce the times of dressing change,improve patient satisfaction,and reduce the scar area of patients with cosmetic suture,which is worthy of clinical application.
