不同血液净化模式在儿童急性肝功能衰竭治疗中的效果及预后分析

摘　　要：目的:探究不同血液净化模式在儿童急性肝功能衰竭治疗中的效果。方法:选取笔者所在医院2017年3月-2020年3月收治的64例急性肝功能衰竭患儿作为研究对象,按照随机数表法分为对照组与观察组,各32例。对照组采用持续血液透析过滤(CVVHDF),观察组在对照组治疗方案基础上增加血浆置换(PE)治疗,对比两组疗效。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗前两组总胆红素、谷丙转氨酶、谷草转氨酶、肌酐、尿素氮、白蛋白指标对比差异无统计学意义(P>0.05),治疗后观察组总胆红素、谷丙转氨酶、谷草转氨酶、肌酐、尿素氮均低于对照组,白蛋白高于对照组,差异均有统计学意义(P<0.05)。结论:CVVHDF联合PE的血液净化模式具有较好的治疗效果,能有效改善患者肝功能,改善患儿预后。Objective:To explore the therapeutic effect of different blood purification modes in children with acute liver failure and its influence on prognosis.Method:A total of 64 children with acute liver failure admitted to the hospital of the author from March 2017 to March 2020 were selected as the research objects.They were randomly divided into the control group and the observation group,32 cases in each group.The control group was treated with continuous hemodialysis filtration (CVVHDF),and the observation group was treated with plasma exchange (PE) on the basis of the control group.The effects of the two groups were compared.Result:The total effective rate in the observation group was higher than that in the control group,the difference was statistically significant (P<0.05).Before treatment,there was no statistically significant difference in the levels of total bilirubin,alanine aminotransferase,aspartate aminotransferase,creatinine,urea nitrogen and albumin between the two groups (P>0.05),after treatment,the levels of total bilirubin,alanine aminotransferase,aspartate aminotransferase,creatinine and urea nitrogen in the observation group were lower than those in the control group,and the albumin index level was higher than that in the control group,the differences were statistically significant (P<0.05).Conclusion:CVVHDF combined with PE blood purification mode has good therapeutic effect,can effectively improve the liver function of patients,improve the prognosis of children.
