摘 要:目的:研究三维彩色血管能量成像联合超声萤火虫成像技术对不同病理分型乳腺肿瘤诊断及预后评估价值。方法:本次研究对象为2015年1月-2017年1月笔者所在医院收治的42例恶性乳腺癌患者及71例乳腺良性病变患者。所有患者分别进行常规超声检查及三维彩色血管能量成像联合超声萤火虫成像技术检查。比较不同病理分型患者的三维彩色血管能量成像及超声萤火虫成像技术参数。比较不同预后患者的三维彩色血管能量成像及超声萤火虫成像技术参数。比较常规超声检测与联合检测的良恶性肿瘤诊断效能。结果:良性患者的VI、FI、VFI、钙化直径均显著低于恶性患者(P<0.05);生存患者的VI、FI、VFI、钙化直径均显著低于死亡患者(P<0.05);患者的VI、FI、VFI、钙化直径与患者的病理分型及生存情况呈现负相关(P<0.05);联合诊断的灵敏度明显高于常规超声检测(P<0.05)。联合检查的ROC曲线分析显示,恶性肿瘤患者的VI、FI、VFI、钙化直径临界值分别为0.27、26.39、2.33、7.91 mm。结论:三维彩色血管能量成像技术联合超声萤火虫成像技术对乳腺肿瘤患者的不同病理分型的判定具有积极的意义,同时通过对患者的血管生成情况及局部钙化情况的判断,对于患者的预后具有积极的预测价值。Objective:To study the diagnostic and prognostic value of three-dimensional color power angiography combined with ultrasound firefly imaging technology in different pathological types of breast cancer.Method:The subjects of this study were 42 patients with malignant breast cancer and 71 patients with benign breast lesions admitted in our hospital from January 2015 to January 2017.All patients were examined by conventional ultrasound and three-dimensional color power angiography and ultrasound firefly imaging technology.The technical parameters of three-dimensional color power angiography and ultrasound firefly imaging technology in patients with different pathological types were compared.The technical parameters of three-dimensional color power angiography and ultrasound firefly imaging technology in patients with different prognosis were compared.The diagnostic efficacy of conventional ultrasound and combined detection in benign and malignant tumors were compared.Result:The VI,FI,VFI and calcification diameter in the benign group were significantly lower than those in the malignant patients (P<0.05).the VI,FI,VFI and calcification diameter in the survival patients were significantly lower than those in the death group (P<0.05).The VI,FI,VFI and calcification diameter of the patients were negatively correlated with the pathological classification and survival (P<0.05).The sensitivity of the combined diagnosis was significantly higher than that of the conventional ultrasound detection (P<0.05).ROC curve analysis of the combined examination showed that the critical values of VI,FI,VFI and calcification diameter in patients with malignant tumor were 0.27,26.39,2.33 and 7.91 mm,respectively.Conclusion:Three-dimensional color power angiography combined with ultrasound firefly imaging technology has a positive significance for the determination of different pathological types of breast cancer patients.At the same time,through the judgment of angiogenesis and local calcification of patients,it has a positive predictive
