摘 要:目的:探究低管电压联合低剂量对比剂MSCT肺血管成像对肺动脉栓塞诊断的价值。方法:选取2019年1月-2020年6月笔者所在医院收治的肺动脉栓塞患者46例,利用随机数字表法分为观察组和对照组,各23例,均采用MSCT肺血管成像进行诊断。对照组采用常规管电压和正常剂量对比剂,观察组采用低管电压和低剂量对比剂,比较两组图像质量、血管强化CT值和辐射量。结果:两组图像质量和图像质量良好率比较差异均无统计学意义(P>0.05)。两组肺动脉主干、左肺动脉和右肺动脉血管强化CT值比较差异均无统计学意义(P>0.05),观察组左肺静脉和右肺静脉血管强化CT值均低于对照组,差异均有统计学意义(P<0.05)。观察组监测扫描CTDvol、肺动脉扫描CTDvol、肺动脉扫描DLP均低于对照组,差异均有统计学意义(P<0.05)。结论:与常规MSCT肺血管成像相比,低管电压联合低剂量对比剂CT在不降低图片质量的前提下,增加了肺动、静脉血管强化对比度,降低了辐射剂量和对比剂剂量,降低了对比剂肾病发病率,值得临床推广。Objective:To explore the value of low tube voltage combined with low dose contrast medium MSCT pulmonary angiography in the diagnosis of pulmonary embolism.Method:A total of 46 patients with pulmonary embolism from January 2019 to June 2020 were selected and randomly divided into the observation group and the control group,23 cases in each group.The control group was treated with conventional tube voltage and normal dose contrast medium,while the observation group was treated with low tube voltage and low dose contrast medium.The Image quality,angiographic enhancement CT value and radiation dose were compared between the two groups.Result:There were no significant differenced in image quality and good image quality rate between the two groups (P>0.05).There were no statistically significant differences between the two groups in terms of enhanced CT values of main pulmonary artery,left pulmonary artery and right pulmonary artery (P>0.05),the enhanced CT values of left pulmonary vein and right pulmonary vein in the observation group were lower than those in the control group,the differences were statistically significant (P<0.05).The monitoring of CTDvol,pulmonary artery scan CTDvol and pulmonary artery scan DLP in the observation group were all lower than those in the control group,the differences were statistically significant (P<0.05).Conclusion:Compared with conventional MSCT pulmonary angiography,low tube voltage and low dose contrast medium CT can increase contrast of pulmonary artery and vein,reduce radiation dose and contrast dose,and reduce the incidence rate of contrast nephropathy without decreasing picture quality,which is worthy of clinical promotion.
