摘 要:目的:探讨早期糖尿病患者左心室亚临床损伤评价中三维斑点追踪技术的应用及临床意义。方法:选取2018年1月-2019年12月笔者所在医院收治的早期糖尿病患者55例作为研究组,同时选取同期在院体检的健康人群55例作为对照组。两组均进行三维斑点追踪技术诊断。比较两组一般资料、实验室检查指标情况、三维斑点追踪技术相关参数情况、左室三维应变情况。结果:两组性别、年龄、心率、收缩压比较差异无统计学意义(P>0.05),研究组BMI、腰臀比、腰围、舒张压、IMT均高于对照组,差异均有统计学意义(P<0.05);两组高密度脂蛋白比较差异无统计学意义(P>0.05),研究组空腹血糖、总胆固醇、低密度脂蛋白、甘油三酯均高于对照组,差异均有统计学意义(P<0.05);两组LVPWT、LVEF、LVESN、IVST比较差异无统计学意义(P>0.05),研究组LAID、E/e’均高于对照组,差异均有统计学意义(P<0.05);两组3D-GRS、3D-GCS、3D-GAS比较差异无统计学意义(P>0.05),研究组3D-GLS高于对照组,差异有统计学意义(P<0.05)。结论:早期糖尿病患者左心室亚临床损伤评价中,三维斑点追踪技术的评价效果较好,值得临床使用。Objective:To explore the application and clinical significance of three-dimensional speckle tracking technique in the evaluation of left ventricular subclinical injury in patients with early diabetes.Method:A total of 55 patients with early diabetes admitted to our hospital from January 2018 to December 2019 were selected as the study group,and 55 healthy people in our hospital during the same period were selected as the control group.Both groups were diagnosed by three-dimensional speckle tracking technique.The general data,laboratory examination indexes,parameters related to 3D speckle tracking technology and 3D strain of left ventricle were compared between the two groups.Result:There were no statistically significant differences in gender,age,heart rate and systolic blood pressure between the two groups (P>0.05),the BMI,waist-hip ratio,waist circumference,diastolic blood pressure and IMT in thestudy group were higher than those in the control group,the differences were statistically significant (P<0.05).There was no significant difference in HDL between the two groups (P>0.05),while fasting blood glucose,total cholesterol,LDL and triglyceride in the study group were higher than those in the control group,the differences were statistically significant (P<0.05).There were no significant differences in LVPWT,LVEF,LVESN and IVST between the two groups (P>0.05),LAI and E/e’ in the study group were higher than those in the control group,the differences were statistically significant (P<0.05).There were no significant differences in 3D-GRS,3D-GCS and 3D-GAS between the two groups (P>0.05),while the 3D-GLS in the study group was higher than that in the control group,the difference was statistically significant (P<0.05).Conclusion:Three-dimensional speckle tracking technique is effective in evaluating left ventricular subclinical injury in patients with early diabetes,which is worthy of clinical application.
