摘　　要：目的:探讨分步法微创胰周脓肿清除术治疗急性重症胰腺炎(acute severe pancreatitis,ASP)合并胰腺脓肿(pancreatic abscess,PA)的效果。方法:回顾性分析2017年1月-2020年6月钦州市第一人民医院和钦州市第二人民医院收治的18例ASP合并PA患者的临床资料。所有患者均经保守治疗或单纯引流治疗无效,并接受分步法微创胰周脓肿清除术治疗。结果:患者平均住院时间为(38.67±15.39)d;接受微创清创的单次操作时间为(51.81±18.91)min;接受清创次数为(4.21±1.70)次;所有患者均治愈,其中11例经治疗后带管出院。患者在治疗过程中均未发生严重的并发症。结论:分步法微创胰周脓肿清除术治疗ASP合并PA,患者的治疗效果良好,并发症少。Objective:To investigate the therapeutic efficacy of fractionated minimally invasive peripancreatic abscess removal in the treatment of acute severe pancreatitis (ASP) complicated with pancreatic abscess (PA).Method:The clinical data of 18 patients with ASP complicated with PA admitted in Qinzhou First People’s Hospital and Qinzhou Second People’s Hospital from January 2017 to June 2020 were retrospectively analyzed.All patients were ineffective after conservative treatment or simple drainage,and were treated with fractionated minimally invasive peripancreatic abscess removal.Result:The average length of hospital stay was (38.67±15.39) d.The single operation time of minimally invasive debridement was (51.81±18.91) min.The number of debridement was (4.21±1.70) times.All patients were cured,and 11 case of them were discharged with tube after treatment.No serious complications occurred in all patients during the treatment.Conclusion:Fractionated minimally invasive peripancreatic abscess removal in the treatment of ASP complicated with PA has good therapeutic effect and few complications.
