两种入路治疗C3型肱骨远端骨折的疗效比较

摘　　要：目的比较肘关节侧方入路和后方入路治疗C3型肱骨远端骨折的临床疗效。方法将37例C3型肱骨远端骨折患者根据手术入路不同分为观察组(15例,采用侧方入路)和对照组(22例,采用后方入路)。记录切口长度、手术时间、术中出血量、肘关节屈伸活动度、前臂旋转活动度及术后并发症情况。末次随访时采用改良Cassebaum评分标准评价肘关节功能。结果患者均获得随访,时间8~16个月。切口长度、手术时间、术中出血量观察组均短(少)于对照组(P<0.05)。末次随访时,两组肘关节屈伸活动度、前臂旋转活动度、肘关节功能优良率、并发症发生率比较差异均无统计学意义(P>0.05)。结论两种入路均能满足复位和固定的需要,临床效果满意,但侧方入路创伤更小,操作更简单。Objective To compare the clinical results of elbow joint lateral approach and posterior approach for type C3 fracture of distal humerus.Methods The 37 cases with type C3 fracture of distal humerus were divided into the observation group(15 cases were treated with lateral approach)and the control group(22 cases were treated with posterior approach),according to different surgical approaches.Length of incision,operation time,intraoperative bleeding volume,extend-flexion range of elbow joint,rotation range of forearm,and postoperative complications were recorded.Elbow function was evaluated by modified Cassebaum scales criterion at the last follow-up.Results All patients were followed up for 8~16 months.Length of incision,operation time and intraoperative bleeding volume in the observation group were shorter(less)than those in the control group(P<0.05).At the last follow-up,the extend-flexion range of elbow joint,rotation range of the forearm,the excellent-good rate of elbow function,the incidence rate of complications had no statistically significant differences between two groups(P>0.05).Conclusions Both approaches can obtain satisfactory clinical outcomes.However,lateral approach results in less trauma and easier operation.
