摘　　要：目的了解低分子肝素类药物临床使用情况,以及医师、药师、护士对其了解程度,为促进该类药物临床合理应用提供参考。方法采用多中心回顾性调查,随机抽取使用低分子肝素类药物出院病历共1097份,对其用药情况进行分析;并通过电子问卷方式调查医师、药师、护士对该类药物的了解程度。结果低分子肝素类药物临床使用率较高,涉及36个临床科室,占总住院患者的60%以上;用药不合理率高达80.04%,主要不合理现象为给药剂量不合理、适应证不适宜和用药频次错误(分别占25.07%、24.53%和13.16%)。医师、药师、护士对该类药物了解情况欠佳。结论低分子肝素类药物的应用存在不合理现象,主要原因是医师、药师、护士对该类药物了解程度不够,因此,有必要采取相关干预措施,促进临床合理应用。Objective To determine the current clinical use of low molecular weight heparin(LMWH)and the knowledge of physicians,pharmacists and nurses on LMWHs,and to provide reference for further use of such drugs.Methods A total of 1097 discharged medical records of LMWHs drug use were randomly sampled and their medication status was retrospectively collected.The knowledge of doctors,pharmacists and nurses on LMWHs was investigated by questionnaire.Results LMWH drugs were wildly used,involving more than 60%of the total hospitalized patients from 36 departments.The irrational rate was as high as 80.04%.The main unreasonable phenomenon was irrational single dose,inappropriate indication and wrong frequency of drug use(accounting for 25.07%,24.53%and 13.16%,respectively).Doctors,pharmacists and nurses had poor knowledge of LMWHs.Conclusion LMWH drugs do have unreasonable usage,mainly because doctors,pharmacists and nurses do not know enough about LMWHs drugs.
