健脾疏肝方联合恩替卡韦治疗乙型肝炎肝硬化的临床效果

摘　　要：目的:探讨健脾补肾方联合恩替卡韦治疗乙型肝炎肝硬化的临床效果。方法:收集2017年6月至2018年12月辽宁省锦州市中心医院感染科收治的60例乙型肝炎肝硬化患者为观察对象,按随机数字表法将入组患者分为治疗组和对照组,每组30例。对照组接受单药恩替卡韦的治疗,治疗组接受在恩替卡韦治疗的基础上加用健脾疏肝方进行治疗。治疗6个月后,通过监测临床症状、肝功能、肝纤维化指标和炎性因子水平的变化,评定两组的治疗效果。结果:治疗后,两组患者临床症状均较治疗前明显缓解(P<0.05),并且治疗组患者临床症状缓解程度明显优于对照组(P<0.05)。两组患者ALT、AST、TBIL、HA、LN、Ⅳ-C、PCⅢ水平和炎性因子IL-6、IL-31、TGF-β1、TNF-α水平以及肝脏影像学指标水平均较治疗前下降,PTA水平较治疗前升高(P<0.05),并且治疗组患者ALT、AST、TBIL、HA、LN、Ⅳ-C、PCⅢ水平和炎性因子水平以及肝脏影像学指标水平明显低于对照组,PTA水平明显高于对照组(P<0.05)。结论:恩替卡韦联合健脾疏肝方治疗和单药恩替卡韦治疗,均可以改善乙型肝炎肝硬化患者预后,前者临床效果更佳,值得在临床上推广应用。Objective:To evaluate the efficacy of entecavir combined with Jianpi Sugan recipe in patients with hepatitis B cirrhosis.Methods:Sixty patients with hepatitis B cirrhosis in our hospital from June 2017 to December 2018 were selected,and were randomly divided into a control group and a treatment group,with 30 cases in each group.The patients in the control group receivedentecavir only,and those in the treatment group received entecavir combined with Jianpi Sugan recipe.After 6 months,the clinical symptoms,liver function,liver fibrosis indicators and inflammatory factor levels were detected to evaluate the treatment effects.Results:After treatment,the clinical symptoms of both groups were significantly relieved(P<0.05),and the improvement of clinical symptoms in the treatment group wasmore obvious(P<0.05).After treatment,the levels of ALT,AST,TBIL,HA,LN,Ⅳ-C and PCⅢ,liver imaging indexesand inflammatory factors such as IL-6,IL-31,TGF-β1 and TNF-αin both groups were decreased,while the PTA level was increased(P<0.05).The changes were more significant in the treatment group(P<0.05).Conclusion:Entecavir combined with Jianpi Sugan recipe has a better therapeutic effect compared to entecavir monotherapy in patients with hepatitis B virus-related cirrhosis,which is worthy of promotion in clinical practice.
