摘　　要：目的:观察湿热痹方联合慢病管理治疗痛风性关节炎的临床疗效。方法:将66例湿热蕴结型痛风患者按随机数字表法分为观察组和对照组。对照组给予双氯芬酸钠和非布司他治疗;观察组在对照组的基础上给予湿热痹方口服联合慢病管理干预。观察并对比两组患者治疗前后在综合疗效、症状体征量化评分、血尿酸(UA)、红细胞沉降率(ESR)、C反应蛋白(CRP)、低密度脂蛋白(LDL)和体质量指数(BMI)方面的变化。结果:总有效率观察组为93.75%,对照组为87.10%;两组患者在疼痛视觉模拟评分法(VAS)评分、UA、ESR、CRP方面,治疗前后对比均显示有效(P<0.05);观察组在降低UA、LDL和BMI方面优于对照组(P<0.05)。结论:湿热痹方联合慢病管理对缓解痛风患者的关节肿痛、UA长期达标、恢复健康状态方面有较好疗效。Objective:To observe the clinical efficacy of Shirebi Decoction(湿热痹方)combined with chronic disease management in the treatment of gout arthritis.Methods:A total of 66 patients with gout of retention of damp-heat type were randomly divided into the observation group and the control group.The control group was treated with diclofenac sodium and febuxostat.On the basis of the treatment of the control group,the observation group was given Shirebi Decoction combined with chronic disease management intervention.The changes of comprehensive curative efficacy,quantitative scores of symptoms and signs,serum uric acid(UA),erythrocyte sedimentation rate(ESR),c-reactive protein(CRP),low-density lipoprotein(LDL)and body mass index(BMI)in the two groups before and after treatment were observed and compared.Results:The total effective rate was93.75%in the experimental group and 87.10%in the control group.In terms of pain visual analogue scale(VAS)score,UA,ESR and CRP,the comparison before and after treatment showed it was effective in both groups(P<0.05).However,the experimental group was superior to the control group in reducing UA,LDL and BMI(P<0.05).Conclusions:Shirebi Decoction combined with chronic disease management has a good effect on relieving joint swelling and pain of gout patients,keeping uric acid up to the standard for a long time,and restoring health.
