宫腹腔镜联合手术在剖宫产瘢痕妊娠治疗中的应用分析

摘　　要：目的探讨宫腹腔镜联合手术作为剖宫产瘢痕妊娠治疗方案的应用价值。方法92例剖宫产瘢痕妊娠患者,依据双盲法分为对照组与观察组,每组46例。对照组实行单一的宫腔镜手术治疗,观察组应用宫腹腔镜联合手术治疗。对比两组的手术耗时、术后住院时间、月经复潮时间、术中出血量及并发症发生情况。结果两组手术耗时比较差异无统计学意义(P>0.05);观察组术后住院时间(2.35±0.81)d及月经复潮时间(36.85±5.01)d明显短于对照组的(2.96±1.06)d、(44.11±7.05)d,术中出血量(101.46±28.71)ml少于对照组的(121.68±33.45)ml,差异具有统计学意义(P<0.05)。观察组并发症发生率为6.52%,低于对照组的21.74%,差异具有统计学意义(P<0.05)。结论在剖宫产瘢痕妊娠治疗中,选择宫腹腔镜联合手术作为治疗方案,效果明显,值得推广使用。Objective To discuss the practical value of combined hysteroscopy and laparoscopy in the treatment of cesarean section scar pregnancy.Methods A total of 92 patients with cesarean section scar pregnancy were divided into control group and observation group according to double-blind method,with 46 cases in each group.The control group was treated with hysteroscopy,and the observation group was treated with combined hysteroscopy and laparoscopy.The operation time,postoperative hospitalization time,menstrual recovery time,intraoperative blood loss and occurrence of complications were compared between the two groups.Results There was no statistically significant difference in operation time between the two groups(P>0.05).The postoperative hospitalization time(2.35±0.81)d and menstrual recovery time(36.85±5.01)d of the observation group were obviously shorter than(2.96±1.06)and(44.11±7.05)d of the control group,and intraoperative blood loss(101.46±28.71)ml was less than(121.68±33.45)ml of the control group,and the difference was statistically significant(P<0.05).The incidence of complications of the observation group was 6.52%,which was lower than 21.74%of the control group,and the difference was statistically significant(P<0.05).Conclusion Hysteroscopy and laparoscopy show obvious effect in cesarean section scar pregnancy,it is worthy of promotion and application.
