抗核抗体联合抗核抗体谱对系统性红斑狼疮的临床诊断价值

摘　　要：目的分析系统性红斑狼疮(SLE)的疾病特点,评价抗核抗体(ANA)联合抗核抗体谱(ANAs)检测的临床诊断价值。方法80例SLE患者作为观察组,80例其他自身免疫学疾病患者作为对照组,两组患者均进行ANA、ANAs测定,比较两组患者的ANA、ANAs阳性检出情况,ANA、ANAs联合检测的阳性检出情况。结果观察组患者的ANA、抗dsDNA抗体、抗Rib-P抗体、抗SSA抗体、抗SSB抗体、抗Ro-52抗体、抗Sm抗体、抗nRNP/Sm抗体阳性检出率分别为80.00%、25.00%、35.00%、60.00%、15.00%、60.00%、25.00%、50.00%,均明显高于对照组的30.00%、0、2.50%、20.00%、5.00%、20.00%、1.25%、10.00%,差异均具有统计学意义(P<0.05)。观察组患者抗dsDNA+Sm抗体、抗dsDNA+Sm抗体+ANA、抗dsDNA+Sm+Rib-P抗体、抗dsDNA+Sm+SSA抗体、抗dsDNA+Sm+Ro-52抗体、抗dsDNA+Sm+nRNP/Sm抗体检测的阳性检出率分别为35.00%、100.00%、55.00%、75.00%、70.00%、65.00%,均高于对照组的1.25%、30.00%、5.00%、20.00%、25.00%、8.75%,差异均具有统计学意义(P<0.05)。结论ANA、ANAs联合检测有较高的阳性检出率,明显有别于其他自身免疫学疾病,有助于SLE疾病的早期诊治,促进患者治疗预后。Objective To analyze the characteristics of systemic lupus erythematosus(SLE),and evaluate the clinical diagnostic value of antinuclear antibody(ANA)combined with antinuclear antibody spectrum(ANAs)detection.Methods There were 80 SLE patients as the observation group,and another 80 patients with other autoimmune diseases served as the control group.Both groups were tested for ANA and ANAs,and the positive detection status of ANA and ANAs in the two groups were compared,and the positive detection status of ANA and ANAs combined detection was compared.Results The positive detection rates of ANA,anti-dsDNA antibody,anti-Rib-P antibody,anti-SSA antibody,anti-SSB antibody,anti-Ro-52 antibody,anti-Sm antibody,and antinRNP/Sm antibody in the observation group were 80.00%,25.00%,35.00%,60.00%,15.00%,60.00%,25.00%and 50.00%,which were obviously higher than 30.00%,0,2.50%,20.00%,5.00%,20.00%,1.25%and 10.00%of the control group,and the difference was statistically significant(P<0.05).The positive detection rates of antidsDNA+Sm antibody,anti-dsDNA+Sm antibody+ANA,anti-dsDNA+Sm+Rib-P antibody,anti-dsDNA+Sm+SSA antibody,anti-dsDNA+Sm+Ro-52 antibody,anti-dsDNA+Sm+nRNP/Sm of the observation group were 35.00%,100.00%,55.00%,75.00%,70.00%and 65.00%,which were all higher than 1.25%,30.00%,5.00%,20.00%,25.00%and 8.75%of the control group,and the difference was statistically significant(P<0.05).Conclusion The combined detection of ANA and ANAs has high sensitivity and specificity,and is obviously different from other autoimmune diseases,thereby helping the early diagnosis and treatment of the disease and promoting the treatment and prognosis of patients.
