实时组织弹性成像弹性定量参数与大鼠免疫性肝纤维化肝组织Ⅰ型胶原纤维和Ⅲ型胶原纤维的相关性研究

摘　　要：目的通过研究实时组织弹性定量参数与免疫性肝纤维化肝组织的Ⅰ型胶原纤维(ColⅠ)和Ⅲ型胶原纤维(ColⅢ)的相关性研究,从分子水平探讨弹性成像定量参数评价肝纤维化分期的可行性。方法共60只SD大鼠,随机选取52只作为实验组,8只作为对照组。实验组通过尾静脉注射猪血清,对照组注射等剂量0.9%氯化钠注射液。自造模第2周起,每2周从实验组与对照组分别随机取6只和1只大鼠进行实时组织弹性成像(RTE)检查,取得12个弹性定量参数。处死大鼠,取肝右叶组织进行病理检查及应用蛋白质印迹法技术定量检测。结果41只大鼠成功造模;RTE弹性定量参数与ColⅠ及ColⅢ的表达水平具有相关性,其中LF Index相关性最强(r=0.614,P<0.05),MEAN次之(r=-0.556,P<0.05),差异有统计学意义。结论RTE弹性定量参数与ColⅠ、ColⅢ表达水平具有相关性,在一定程度上可以评价肝组织的纤维化程度。Objective This study analyzed the correlation between real-time tissue elastography(RTE)quantitative parameters and Col Ⅰ and Col Ⅲ in immune hepatitis fibrosis,exploring the feasibility of elastography quantitative parameters to evaluate the stage of fibrosis.Methods Sixty SD rats were selected,52 were experimental group and 8 were control group.The experimental group was injected with 0.5 mL swine serum through tail vein,while control group was injected with same dose of saline twice a week for 12 weeks.Six rats from experimental group and,rat from control group were randomly selected to undergo RTE every two weeks,obtaining 12 quantitative parameters of elasticity.Then rats were sacrificed and the right lobe of liver sample were taken to pathological examination and Western-blot.Results Here existed correlations among quantitative parameters of RTE and the level of Col Ⅰ and Col Ⅲ.Among 12 quantitative parameters,LF Index showed highest correlation(r=0.614,P<0.05),MEAN unveiled the second highest correlation(r=-0.556,P<0.05).Conclusion Quantitative parameters showed correlations with the level of Col Ⅰ and Col Ⅲ.They can evaluate the stage of fibrosis,providing guidance for the accurate evaluation and treatment of immune hepatitis fibrosis.
