摘 要:目的:系统评价经皮电刺激(TES)治疗膝骨关节炎(KOA)的有效性与安全性,为临床治疗提供参考。方法:计算机检索在2019年10月之前公开发表在CNKI、万方、维普、Embase、PubMed、CBM、Cochrane图书馆中关于TES治疗KOA的随机对照研究试验。由2位评价者独立进行文献数据提取,按Cochrane协作网标准进行质量评价,用RevMan 5.3软件进行Meta统计分析。结果:按照纳入标准最终纳入6篇随机对照试验,共991例患者,其中试验组502例,对照组489例,比例接近1∶1。Meta分析结果显示,试验组患者总体有效率[RR=0.08,95%CI(0.01,0.61),P=0.02]、视觉模拟疼痛(VAS)评分[MD=1.47,95%CI(1.16,1.77),P<0.001]、不良反应发生率[RR=0.08,95%CI(0.01,0.61),P=0.02]均显著优于对照组,差异均有统计学意义;在Lysholm膝关节功能评分量表[MD=3.71,95%CI(-1.59,9.01),P=0.17]方面,2组差异无统计学意义。结论:TES在治疗KOA方面具有明显优势,可以有效减轻患者关节疼痛,改善关节功能,提高生活质量。Objective:To systematically evaluate the efficacy and safety of transdermal electrical stimulation(TES)in the treatment of knee osteoarthritis(KOA).Methods:A computerized systematic search of randomized controlled trials published in CNKI,Wanfang,VIP,Embase,PubMed,CBM,Cochrane libraries on the treatment of KOA by TES prior to October 2019.Literature data were extracted and reviewed independently by two reviewers,quality evaluation was conducted following the Cochrane collaborative standards,and Meta-analysis was performed using RevMan 5.3 software.Results:According to the inclusion criteria,6 randomized controlled trials with a total of 991 patients were included,including 502 in the experimental group and 489 in the control group,with a ratio of 1∶1.Meta-analysis results showed that the overall effective rate[RR=0.08,95%CI(0.01,0.61),P=0.02],visual analogue pain(VAS)score[MD=1.47,95%CI(1.16,1.77),P<0.001]and incidence of adverse reactions[RR=0.08,95%CI(0.01,0.61),P=0.02]in the experimental group were significantly better than those in the control group.Lysholm knee function evaluation function score scale[MD=3.71,95%CI(-1.59,9.01),P=0.17]showed no significant difference between the two groups.Conclusion:Transdermal electrical stimulation has significant advantages in the treatment of KOA,which can effectively reduce joint pain,improve joint function and quality of life.
