从针灸临床复合干预体系探讨细胞因子风暴的防治思路

摘　　要：探讨细胞因子风暴(CS)的发生发展机制及其防治手段,梳理艾灸、刮痧、放血等疗法治疗传染性疾病的历史源流,首次提出针灸临床复合干预体系的概念,即以经络腧穴理论为指导,运用刮痧、放血、艾灸等干预方法,针对不同疾病以及同一疾病的不同阶段,选择不同介入时机施以综合治疗的干预体系。根据上述干预方法免疫作用机制的现代研究结果,提出“关键因子耗竭”可能是针灸临床复合干预体系减少或消除CS的潜在机制,即上述干预方法可通过局部皮肤损伤与炎症反应募集并消耗全身炎症反应过程中的“关键因子”,使得机体在遭受重症感染过程中产生CS所必须的“关键因子”无法达到所需阈值,导致“关键因子耗竭”而使得CS的出现概率降低甚至消失,实现减少或消除CS的可能,从而减轻或延缓多器官功能衰竭综合征(MODS)的发生发展。To explore the mechanism of cytokine storm(CS)and its prevention and treatment methods,and to sort out the historical origin of moxibustion,scraping and bloodletting in the treatment of infectious diseases.The concept of acupuncture clinical complex intervention system was put forward for the first time.The system was guided by the theory of meridians and acupoints,using scraping,bloodletting and moxibustion.According to different diseases and different stages of the same disease,a variety of intervention methods and different intervention timing were selected.According to the modern research results of the immune mechanism of the intervention means,"critical factor depletion"may be the potential mechanism of reducing or eliminating CS in acupuncture clinical complex intervention system.The above intervention methods can recruit and consume the"critical factor"in the systemic inflammatory response through local skin injury and inflammatory response.So that,the"critical factor"cannot reach the required threshold during severe infections.This leads to the depletion of key factors and makes the probability of CS decrease or even disappear.Finally,the possibility of reducing or eliminating CS can be realized,and the occurrence and development of multiple organ dysfunction syndrome(MODS)could be alleviated/delayed.
