摘　　要：目的了解某院近5年来手卫生依从性监测情况及2020年新型冠状病毒肺炎疫情下手卫生执行情况。方法对某三级甲等医院2016年1月-2020年8月所有病区及手术室等医务人员(HCWs)手卫生依从性监测结果进行分析。结果2016年1月-2020年8月平均手卫生依从率为75.95%,近5年HCWs手卫生依从率呈逐年上升趋势,差异有统计学意义(P<0.05)。近5年不同职业类别HCWs平均手卫生依从率最高为护士(78.61%),其次为工勤(73.56%)、医生(72.59%)、医技(67.15%),最低为其他(66.16%);不同职业类别HCWs平均手卫生依从率比较,差异有统计学意义(P<0.05)。近5年不同手卫生指征HCWs平均手卫生依从率最高为暴露患者体液风险后(88.53%),最低为接触患者周围环境后(63.23%);不同手卫生指征HCWs平均手卫生依从率比较,差异有统计学意义(P<0.05)。2020年1月-8月HCWs平均手卫生依从率为84.16%,其中2月HCWs手卫生依从率最高,为88.77%;2020年2月-8月HCWs手卫生依从率呈下降趋势,差异有统计学意义(P<0.05)。结论某院近5年HCWs手卫生依从率逐步上升,但仍需要加强手卫生管理,尤其是在突发公共卫生事件后。OBJECTIVE To investigate the compliance with hand hygiene compliance in a hospital in the past 5 years and during the COIVD-19 pandemic in 2020.METHODS The monitoring results of hand hygiene compliance in all healthcare-associated workers(HCWs)in wards and operating rooms of a third-grade and class-A hospital from January 2016 to August 2020 were analyzed.RESULTS The average hand hygiene compliance rate from January 2016 to August 2020 was 75.95%.The hand hygiene compliance rate of HCWs had been increasing year by year in the past 5 years,and the difference was significant(P<0.05).Among different occupational categories,nurses have the highest average rate of hand hygiene compliance(78.61%),followed by sanitation workers and care workers(73.56%),doctors(72.59%),technicians(67.15%)and others(66.16%).The average rates of hand hygiene compliance rate in different occupational categories were compared,and the differences was significant(P<0.05).In the past 5 years,the average hand hygiene compliance rate in HCWs with different hand hygiene indications was the highest after exposure to the patient’s body fluid(88.53%),and that was the lowest after exposure to the patient’s surrounding environment(63.23%).The differences of the average hand hygiene compliance rates among HCWs with different hand hygiene indications were significant(P<0.05).From January to August 2020,the average hand hygiene compliance rate in HCWs was 84.16%,of which the HCWs hand hygiene compliance rate was the highest in February,with the rate of 88.77%.The compliance rate of HCWs hand hygiene showed a downward trend from February to August 2020,and the difference was significant(P<0.05).CONCLUSION The compliance rate of HCWs hand hygiene in a hospital has gradually increased in the past 5 years,but hand hygiene management still needs to be strengthened,especially after public health emergencies.
