摘 要:目的探讨吸痰后两种负压吸引管冲洗方法的临床效果。方法选取2019年1月-2019年12月入住北京某三级甲等医院呼吸科留置人工气道的住院患者100例,随机分为试验组和对照组各50例,试验组在吸痰后采用改进负压吸引管冲洗法,对照组采用常规负压吸引管冲洗法。比较24h后两组瓶装生理盐水细菌培养阳性率,24h后两组负压吸引管接头处3 cm内细菌培养阳性率,护士在为两组患者吸痰后手部细菌菌落计数合格率。结果24 h后试验组的瓶装生理盐水细菌培养阳性率为16.0%低于对照组的46.0%(P<0.05),24h后两组负压吸引管接头处3 cm内细菌培养阳性率无差异,吸痰后试验组护士手部细菌菌落计数的合格率为44.0%高于对照组的24.0%(P<0.05)。结论改进负压吸引管冲洗法可以减少冲洗用瓶装生理盐水的污染,减少吸痰操作对护士手的污染,对减少医院感染和护士职业暴露有积极意义。OBJECTIVE To explore the clinical effect of two methods of washing suction tube after sputum suction.METHODS A total of 100 patients with indwelling artificial airway in respiratory department of a third—grade hospital in Beijing during Jan.to Dec.2019 were recruited and randomly divided into two groups,50 patients in each group.After sputum suction,the improved negative pressure suction tube washing method were performed in the test group,whereas the control group adopted the conventional negative pressure suction tube washing method.The positive rate of bacterial culture of bottled normal saline and innerwall of negative pressure suction pipe,which was 3 cm away from unions,and the qualification rate of bacterial colony count in hands of nurses after sputum suction were compared between the two groups after 24 hours.RESULTS After 24 hours,the positive rate of bacterial culture of bottled normal saline in the test group was significantly lower than that in the control group(P<0.05).After 24 hours,there was no difference in the positive rate of bacterial culture within 3 cm of the joint between the two groups.The qualified rate of hand bacterial colony count in the test group was significantly higher than that in the control group(P<0.05).CONCLUSION The improved washing method of negative pressure suction tube can reduce the contamination of bottled normal saline for washing,and reduce the contamination of nurses’hands after suction operation,which may have positive significance in reducing hospital infection and occupational exposure of nurses.
