摘  要:目的探讨对中孕早期胎儿实施四维超声时间-空间关联成像技术(STIC)检查对心脏畸形的诊断价值。方法选取2019年2月至2020年4月济源市人民医院收治的超声心动图检查提示胎儿心脏畸形确诊或疑似孕妇215例,对其实施四维超声STIC检查,并以尸检病理结果或新生儿超声诊断结果作为评估标准,对四维超声STIC的诊断价值进行评估。结果本研究中,215例胎儿经常规超声心动图共计检出心脏畸形196例(91.16%),5例漏诊(2.33%),14例误诊(6.51%);四维超声STIC共检出心脏畸形208例(96.74%),2例漏诊(0.93%),5例误诊(2.33%);四维超声STIC对于中孕期胎儿心脏畸形的检查准确率明显高于常规超声心动图,差异有统计学意义(P<0.05)。结论采用四维超声STIC技术进行中孕期畸形检查,与常规二维超声相比可有效提高对胎儿心脏畸形的检查准确率,为孕期指导提供准确依据,对于提高出生人口质量,保障母婴安全具有十分积极的作用。Objective To investigate the diagnostic value of four-dimensional ultrasound timespace correlation imaging technology(STIC)for the diagnosis of cardiac malformations.Methods From February 2019 to April 2020,215 cases of fetal cardiac malformation confirmed or suspected pregnant women were diagnosed by echocardiography in Jiyuan People's Hospital,and four-dimensional ultrasound STIC examination was carried out.As an evaluation criterion,the diagnostic value of four-dimensional ultrasound STIC was evaluated.Results In this study,a total of 215 fetuses with cardiac abnormalities were detected by conventional echocardiography in 196 cases(91.16%),5 cases were missed(2.33%),and 14 cases were misdiagnosed(6.51%);4D ultrasound STIC detected a total of heart 208 cases of malformations(96.74%),2 cases of missed diagnosis(0.93%),5 cases of misdiagnosis(2.33%);the accuracy of four-dimensional ultrasound STIC for the detection of fetal heart malformations in mid-pregnancy was significantly higher than that of conventional echocardiography,the difference was significant(P<0.05).Conclusions The use of four-dimensional ultrasound STIC technology for midpregnancy deformity examination can effectively improve the accuracy of fetal heart malformation examination compared with conventional two-dimensional ultrasound,provide accurate basis for pregnancy guidance,and improve the quality of the birth population and ensure the safety of mothers and infants.
