经纤维支气管镜灌洗清开灵口服液治疗重症肺炎(痰热壅肺证)的临床研究

摘　　要：目的观察经纤维支气管镜灌洗清开灵口服液治疗重症肺炎(痰热壅肺证)的临床疗效。方法将70例重症肺炎(痰热壅肺证)患者随机分为治疗组与对照组两组各35例。两组均予西医常规治疗,对照组采用经纤维支气管镜灌洗0.9%氯化钠注射液治疗,治疗组采用经纤维支气管镜灌洗清开灵口服液治疗。两组均治疗2周。结果两组治疗后咳嗽评分、咯痰评分、喘息评分、白细胞(WBC)计数、C反应蛋白(CRP)、降钙素原(PCT)、动脉二氧化碳分压(PaLCO2)较治疗前降低(P<0.05),且治疗后治疗组低于对照组(P<0.05);两组治疗后包括动脉血氧分压(PaO2)、血氧饱和度(SaO2)较治疗前升高(P<0.05),且治疗后治疗组高于对照组(P<0.05);治疗组总有效率高于对照组(P<0.05)。结论经纤维支气管镜灌洗清开灵口服液治疗重症肺炎(痰热壅肺证),可显著改善临床症状及呼吸功能,抑制炎症因子表达,提高临床疗效。Objective:To observe the clinical efficacy of Qingkailing Oral Liquid lavag’e through fiberoptic bronchoscope in the treatment of severe pneumonia(syndrome of phlegm-heat obstructing the lung).Methods:Seventy patients with severe pneumonia(syndrome of phlegm-heat obstructing the lung)were randomly divided into two groups,35 cases in each group.Both groups were treated with western medicine.The control group was treated with 0.9%sodium chloride injection by fiberoptic bronchoscope,and the treatment group was treated with Qingkailing Oral Liquid by fiberoptic bronchoscope.Both groups were treated for 2 weeks.Results:The cough score,cough score,wheezing score,WBC,CRP,PCT and PaCO2 of the two groups were lower than those before treatment(P<0.05),and the results in the treatment group were lower than those of the control group(P<0.05).The PaO2 and SaO2 were increased in both groups after treatment(P<0.05),and the results in the treatment group were higher than those of the control group(P<0.05).The total effective rate of the treatment group was higher than that of the control group(P<0.05).Conclusion:Treatment of severe pneumonia(syndrome of phlegm-heat obstructing the lung)by fiberoptic bronchoscopic lavage of Qingkailing Oral Liquid can significantly improve clinical symptoms and respiratory function,inhibit the expression of inflmmatorV factors,and improve clinical efficacy.
