托毒益气养阴汤治疗肛周坏死性筋膜炎(毒盛正虚证)的临床研究

摘　　要：目的观察托毒益气养阴汤治疗肛周坏死性筋膜炎(毒盛正虚证)的临床疗效。方法将66例患者随机分为治疗组与对照组各33例,对照组采用西医常规治疗,治疗组在对照组基础上采用内服托毒益气养阴汤治疗。两组均治疗2周。结果两组治疗后创面疼痛、病变范围、全身症状、坏死性筋膜炎实验室风险指数(LRINEC)评分及白细胞计数(WBC)、C反应蛋白(CRP)较治疗前均降低(P<0.05),且治疗组低于对照组(P<0.05);两组治疗后血红蛋白(Hb)较治疗前均升高(P<0.05),且治疗组高于对照组(P<0.05);治疗组创面表皮生长时间、创面愈合时间短于对照组(P<0.05);治疗组住院费用低于对照组(P<0.05);治疗组总有效率高于对照组(P<0.05)。结论托毒益气养阴汤治疗肛周坏死性筋膜炎(毒盛正虚证),能够抑制炎症因子表达,增加Hb含量,减轻肛周疼痛、肿胀范围及全身症状,促进创面表皮生长和创面愈合,降低住院费用,提高临床疗效。Objective:To observe the clinical efficacy of Tuodu Yiqi Yangyin Decoction in the treatment of perianal necrotizing fasciitis of toxin excessiveness and healthy qi deficiency type.Methods:A total of 66 patients with perianal necrotizing fasciitis of toxin excessiveness and healthy qi deficiency type were randomly divided into two groups,33 cases in each group.The control group was treated with conventional western medicine,and the treatment group was treated with oral Tuodu Yiqi Yangyin Decoction on the basis of the control group.Both groups were treated for 2 weeks.Results:After treatment,the wound pain,lesion range,systemic symptoms,Lrinec scores,WBC and CRP levels in both groups were lower than before treatment(P<0.05),and the results in the treatment group were lower than those of the control group(P<0.05).The levels of Hb increased after treatment in both groups than before treatment(P<0.05),and the treatment group was higher than the control group(P<0.05).The epidermal growth time and wound healing time of the treatment group were shorter than the control group(P<0.05).The hospitalization cost of the treatment group was lower than that of the control group(P<0.05).The total effective rate of the treatment group was higher than that of the control group(P<0.05).Conclusion:Tuodu Yiqi Yangyin Decoction for perianal necrotizing fasciitis of toxin excessiveness and healthy qi deficiency type can inhibit the expression of inflmmatory factors,increase the content of hemoglobin,reduce the perianal pain,swelling range and systemic symptoms,promote wound epidermal growth and wound healing,reduce hospitalization cost,and improve clinical efficacy.
