中医集束化治疗对呼吸机依赖患者的影响研究

摘　　要：目的研究以金水相生为治则的中医集束化治疗对呼吸机依赖的治疗作用。方法纳入呼吸机依赖患者85例,随机分为对照组44例与治疗组41例,对照组采取西医常规治疗,治疗组在此基础上加用以金水相生为治则的中医集束化治疗方案,疗程7 d,检测两组患者单次脱机时间、28 d成功撤机率,氧合指数、痰液中SP-A水平的差异。结果治疗组痰液中SP-A水平第7天与第1天相比均升高(P<0.05),较对照组升高明显(P<0.05)。对照组痰液中SP-A水平前后变化不大(P>0.05)。两组第7天与第1天相比,氧合指数均有不同程度改善(P<0.05),治疗组在第7天氧合指数较对照组更高(P<0.05)。两组28 d内顺利撤机率无统计学差异(P>0.05)。结论以金水相生为治则的中医集束化治疗可提高呼吸机依赖患者的氧合指数,缩短单次脱机时间,提高痰中SP-A含量。SP-A水平升高或可作为中医理论中肺阴滋长的一种微观表现。Objective:To study the therapeutic effect of traditional Chinese medicine(TCM)cluster therapy based on the principle of"Jinshui Xiangsheng(mutual promotion between lung and kidney)"on ventilator dependent patients.Methods:A total of 85 ventilator dependent patients were randomly divided into the control group(n=44)and treatment group(n=41).The control group was treated with conventional western medicine,while the treatment group was treated with TCM cluster therapy based on the principle of"Jinshui Xiangsheng(mutual promotion between lung and kidney)’additionally;the treatment course was 7 days.The difference in single weaning time,28 d successful weaning rate,oxygenation index,and SP-A level in sputum between the two groups were tested.Results:The SP-A level in sputum of the treatment group increased on the 7 th day compared with the 1 st day(P<0.05),and the increase was significantly higher than that of the control group(P<0.05).The SP-A level in sputum of the control group had no remarkable changes(P>0.05).Compared with the 1 st day,the oxygenation index of these two groups on the 7 th day were improved in varying degrees respectively(P<0.05),and the oxygenation index of the treatment group was higher than that of the control group on the 7 th day(P<0.05).There was no apparent difference between the two groups in the 28 d successful weaning rate(P>0.05).Conclusion:TCM cluster therapy based on the principle of"Jinshui Xiangsheng(mutual promotion between lung and kidney)"can increase the oxygenation index of ventilator dependent patients,shorten the single weaning time,and increase the concentration of SP-A in sputum.The rise of SP-A level may be regarded as a microcosmic appearance of lung yin growth in TCM theory.
