摘 要:目的探讨儿童重症腺病毒肺炎的危险因素。方法抽取2019年1月至12月于厦门市儿童医院住院治疗的腺病毒肺炎患儿190例,其中轻症147例,重症43例。重症腺病毒肺炎危险因素行单因素卡方检验及多因素Logistic分析。结果190例腺病毒肺炎患儿中,轻症者与重症者年龄(≤2岁)、喘息、合并真菌感染、合并其他病毒感染、贫血、肝功能损害、心肌损害、肺不张、胸腔积液、气胸、气道狭窄、闭塞性细支气管炎比较,差异有统计学意义(P<0.05)。Logistic分析显示,心肌损害95%CI 1.638~39.611,P=0.01;肺不张95%CI1.192~29.524,P=0.03。结论肺不张及心肌损害为重症腺病毒肺炎的独立危险因素。Objective To investigate the risk factors of severe adenovirus pneumonia in children.Methods A total of 190 children with adenovirus pneumonia who were hospitalized in Xiamen Children’s Hospital from January 2019 to December 2019 were selected.Among them,147 were mild and 43 were severe.The risk factors of severe adenovirus pneumonia were analyzed by univariate chi-square test and multivariate Logistic statistical analysis.Results Among the 190 children with adenovirus pneumonia,there were significant difference in age(age 2 or less),breathing,the fungal infection,the combining other virus infections,anemia,liver function damage,myocardial damage,atelectasis,pleural effusion,pneumothorax,airway stenosis,occlusive bronchiolitis between mild and severe patients(P<0.05).Logistic analysis showed that myocardial damage was 95%CI 1.638-39.611,P=0.01;and atelectasis was 95%CI1.192-29.524,P=0.03.Conclusions The independent risk factors for severe adenovirus pneumonia are atelectasis and myocardial damage.
