百令胶囊联合厄贝沙坦治疗CGN的疗效研究

摘　　要：目的探讨百令胶囊联合厄贝沙坦对慢性肾小球肾炎(CGN)的疗效。方法选取2018年9月至2019年10月沈阳二四二医院收治的126例CGN患者为研究对象,分为联合治疗组和对照组各63例。对照组给予厄贝沙坦(每次150 mg,每天1次)治疗,联合治疗组在对照组基础上加用百令胶囊(每次2.0 g,每天3次)进行治疗,疗程均为12周。观察两组的临床疗效、24 h尿蛋白量及肾功能变化情况,并比较两组治疗前后血清超敏-C反应蛋白(hs-CRP)、白细胞介素(IL)-6、IL-35和肿瘤坏死因子-α(TNF-α)水平。结果治疗后两组24 h尿蛋白量、血尿素氮(BUN)、血肌酐(SCr)和肾小球滤过率(GFR)水平均较治疗前明显下降,且联合治疗组较对照组更低(P<0.05)。联合治疗组总有效率明显高于对照组(92.06%vs.79.37%),差异有统计学意义(P<0.05)。治疗后联合治疗组hs-CRP、IL-6和TNF-α水平均降低,IL-35水平升高,较对照组变化更明显(P<0.05)。结论百令胶囊联合厄贝沙坦可降低蛋白尿水平,进而改善肾功能,其作用可能与下调促炎性因子表达、上调抑炎性因子水平有关。Objective To explore the efficacy of Bailing capsule combined with Irbesartan on chronic glomerulonephritis(CGN).Methods A total of 126 CGN patients admitted to Shenyang 242 Hospital from September 2018 to October 2019 were selected as the research objects and divided into the combination treatment group and the control group with 63 cases in each group.The control group was treated with Irbesartan(150 mg each time,once a day),and the combined treatment group was treated with Bailing capsules(2.0 g each time,3 times a day)on the basis of the control group.The course of treatment was 12 weeks.Observed the clinical efficacy,24-hour urine protein and renal function changes of the two groups,and compared the levels of high-sensitivity-C-reactive protein(hs-CRP),interleukin(IL)-6,IL-35 and tumors necrosis factor-α(TNF-α)before and after treatment.Results After treatment,the levels of urine protein,blood urea(BUN),blood creatinine(SCr)and glomerular filtration rate(GFR)levels in the two groups at 24 h after treatment were significantly lower than before treatment,and the combined treatment group was lower than the control group(P<0.05).The total effective rate of the combined treatment group was significantly higher than that of the control group(92.06%vs.79.37%),the difference was statistically significant(P<0.05).After treatment,the levels of hs-CRP,IL-6 and TNF-αin the combined treatment group decreased,while the level of IL-35 increased,which was more significant than the control group(P<0.05).Conclusion Bailing capsule combined with Irbesartan could reduce proteinuria level and improve renal function.Its effect may be related to down-regulating the expression of pro-inflammatory factors and up-regulating the level of anti-inflammatory factors.
