摘　　要：目的:了解某三级甲等综合医院(以下简称"该院")西医师开具中成药的现状,促进临床合理用药。方法:通过医院信息管理系统,收集2019年7月该院西城院区西医师开具的门诊处方和住院医嘱数据,对其中的中成药和西药数据进行分类统计和对比分析。结果:2019年7月,该院西城院区门诊中成药处方总金额为652.73万元,其中门诊西医师开具的中成药处方金额为597.36万元(占91.52%);门诊共有679名西医师开具了处方,其中483名西医师开具了中成药处方(占71.13%);门诊西医师共开具处方243019张,其中中成药处方43652张(占17.96%);由西医师出诊的门诊科室中,平均中成药处方金额占比为12.38%。结论:门诊是医师开具中成药的主要场所,>90%的中成药由普遍缺乏相关中医药基础理论的西医师使用,存在一定的不合理用药风险。综合医院应规范门诊中成药处方的开具,促进中成药科学合理使用,保障患者用药安全。OBJECTIVE:To investigate the current situation of prescription of Chinese patent medicine in doctors of western medicine in a gradeⅢlevel A general hospital(hereinafter referred to as"this hospital"),promoting clinical rational drug application.METHODS:Through Hospital Information System,data of outpatient prescriptions and inpatient medical orders prescribed by doctors of western medicine in this hospital in Jul.2019 were collected,classified statistics and comparative analysis were conducted on the Chinese patent medicine and western medicine data.RESULTS:In Jul.2019,the total amount of outpatient prescriptions of Chinese patent medicine was 6.5273 million yuan in this hospital,of which 5.9736 million yuan(91.52%)was prescribed by doctors of western medicine;totally 679 doctors of western medicine had prescribed prescriptions,of which 483 had prescribed prescriptions of Chinese patent medicine(71.13%);there were 243019 prescriptions were prescribed by outpatient doctors of western medicine,of which 43652 were prescriptions of Chinese patent medicine(17.96%);in outpatient departments visited by doctors of western medicine,the average cost of prescription of Chinese patent medicine accounted for 12.38%.CONCLUSIONS:Outpatient is the main place for doctors to prescribe Chinese patent medicine,>90%of Chinese patent medicine are used by doctors of western medicine who generally lack the relevant basic theories of Chinese medicine,which has a certain risk of irrational drug application.General hospital should standardize the prescription of Chinese patent medicine in outpatient,promote the scientific and rational application of Chinese patent medicine,and ensure safe medication.
