摘  要:目的:探讨冠心病患者血液中可溶性生长刺激表达基因2蛋白(sST2)和中性粒细胞/淋巴细胞比值(NLR)变化及与患者冠脉病变程度的相关性。方法:选取心内科住院患者389例,依CASS诊断标准分为非冠心病组(Non-CHD组,n=72)和冠心病组(CHD组,n=317),CHD组患者根据不同的疾病类型分为非ST段抬高型心肌梗死组(NSTEMI组,n=76)、ST段抬高型心肌梗死组(STEMI组,n=91)、不稳定型心绞痛组(UAP组,n=63)和稳定型心绞痛组(SAP组,n=87);再依据Gensini评分分为轻度组(LD组,Gensini≤30分,n=121)、中度组(MD组,30<Gensini<60,n=101)和重度组(SD组,Gensini≥60分,n=95)。另选同期门诊健康体检者95例为对照组(Control组)。采用ELISA法和全自动生化分析仪分别检测各组研究对象血液中sST2和NLR水平,分析冠心病患者血液sST2和NLR水平变化以及不同类型和不同严重程度冠心病患者血液sST2和NLR水平差异。结果:与Control组比较,Non-CHD组和CHD组血液sST2和NLR水平均明显增高(P均<0.05)。与Non-CHD组比较,CHD组血液sST2和NLR水平均明显增加(P均<0.01),冠心病不同类型患者四个亚组间sST2水平为STEMI组>NSTEMI组>UAP组>SAP组,而NLR水平为STEMI组>UAP组>NSTEMI组>SAP组(P均<0.01)。不同严重程度三个亚组患者血液中sST2和NLR水平呈SD组>MD组>LD组趋势(P均<0.01)。sST2和NLR水平与冠脉病变严重程度呈显著正相关(r=0.77、0.72,P均<0.01)。结论:冠心病患者血液中sST2和NLR水平升高,且其水平与冠脉病变严重程度有关。Objective:To investigate of correlationship between soluble growth stimulation expressed gene 2(sST2)and neutrophil-lymphocyte ratio(NLR)and coronary lesion degree in patients with coronary heart disease(CHD).Method:A total of 389 patients with heart pain were respectively divided to Non-CHD group(n=72 cases)and CHD group(n=317 cases),which was continuely divided into Non-ST-elevation myocardial infarction group(NSTEMI group,n=76 cases),ST-elevation myocardial infarction group(STEMI group,n=91 cases),unstableangina pectoris group(UAP group,n=63 cases)and stable angina pectoris group(SAP group,n=87 cases)according to diagnostic criteria come from coronary artery surgery study.And according to Gensini scores estimated by results of percutaneous coronary intervention(PCI),the CHD patients were respectively divided into light degree group(LD group,Gensini≤30,n=121 cases),moderate degree group(MD group,30<Gensini<60,n=101 cases)amd severe degree group(SD group,Gensini≥60,n=95 cases).Another 95 healthy examinations come from clinic service were enrolled into control group.The level of sST2 and NLR in blood of research subjects come from every group were respectively detected by ELISA test and automatic biochemical blood analyzer before and after PCI.And the difference of level of sST2 and NLR were statistcsed and the correlationship between sST2 and NLR level changes and CHD was analyzed.Results:Compared with control group,the levels of sST2 and NLR were all obviously increased not only in Non-CHD group but also in CHD group(P of all<0.05,respectively).Moreover,the increasing was much more in CHD group than in Non-CHD group(P<0.01).Compared with Non-CHD group,there were all notably modified from large to small,which was changed with STEMI group>NSTEMI group>UAP group>SAP group about the level of sST2,and with STEMI group>UAP group>NSTEMI group>SAP group about level of NLR(P of all<0.01).Another the level of sST2 and NLR in patients with different coronary lesion degree were changed form high to low according to SD
